    Application of two-stage citrate anticoagulation in continuous daytime renal replacement therapy with calcium replacement fluid

    Objective To investigate the anticoagulant effect and safety of two-stage regional citrate anticoagulation regimen used in daytime continuous renal replacement therapy with calcium-containing replacement fluid. Methods A total of 96 patients who received daytime renal replacement therapy in a tertiary hospital in Wuhan were selected and randomly divided into experimental group and control group by 2:1. Two-stage citrate anticoagulation regimen was used in experimental group and traditional citrate anticoagulation regimen was used in control group. The incidence of tube plugging,coagulation grade of the filter and vein pot after disoperation,calcium ion concentration 2 h after beginning,and relative blood gas index and total calcium/ionic calcium ratio at the end of treatment were compared between the two groups. Results The incidence of tube plugging in the experimental group was lower than that in the control group,the difference was statistically significant(χ2=4.980,P<0.05). The coagulation grade of the vein pot in the experimental group was significantly better than the control group,the difference was statistically significant(Z=3.556,P<0.001).The 2 h venous calcium concentration of experimental group was lower than that of control group,and the difference was statistically significant(t=7.418,P<0.001). At the end of treatment,the relevant blood gas markers in both groups were within the normal range,total calcium/ionic calcium<2.5,the difference was not statistically significant(P>0.05). Conclusion Two-stage regional citrate anticoagulation can reduce the incidence of tube clotting in continuous renal replacement therapy with calcium replacement fluid and improve the anticoagulant effect of vein pot. At the same time,the scheme does not increase the risk of citrate accumulation,so it is a safe anticoagulant choice.

    Study on the application effect of early activity based on structured quality improvement in ICU patients after liver transplantation

    Objective To study the effect of early activity based on structured quality improvement in ICU patients after liver transplantation. Methods Liver transplantation patients admitted to the ICU of a tertiary class A hospital in Beijing from September 2019 to May 2022 were selected as the study objects. Sixty-three patients from January 2021 to May 2022 were included into experimental groups according to the order of surgery. Early activities based on structured quality improvement were carried out. A total of 62 patients from September 2019 to December 2020 served as the control group and received routine early activities. The risk level of pressure injury after ICU and discharge,the level of daily self-care ability after ICU and discharge,postoperative pain,and the incidence of deep venous thrombosis of lower limbs were compared between the two groups. Results The risk level of pressure injury out of ICU in experimental group was lower than that in control group,and the level of daily living activity ability out of ICU and out of hospital was higher than that in control group,the difference was statistically significant(P<0.05). 13 patients(2.06%)had postoperative pain in the experimental group and 12 patients(1.93%)in the control group,the difference was not statistically significant(P>0.05). No deep vein thrombosis occurred in the experimental group,and 1 case(1.6%)in the control group(P>0.05). Conclusion The early activity nursing plan of ICU patients after liver transplantation based on structured quality improvement is helpful to reduce the risk of pressure injury,improve the self-care ability of patients after liver transplantation,and improve the quality of life of patients.

    Objective To understand the experience of ICU nurses performing early functional exercise for severe patients,so as to provide reference for promoting the the early prevention and treatment of ICU-acquired weakness(ICU-AW). Methods Using the objective sampling method,a semi-structured in-depth interview was conducted with 12 nurses who met the inclusion criteria in the comprehensive intensive care unit of a tertiary Grade A hospital in Beijing from January to April 2022. Colaizzi phenomenological research method was used to analyze the interview data. Results Four themes were obtained,including benefiting from effective doctor-nurse-patient communication and cooperation,lack of implementation norms and management systems,facing with difficulties in implementation and nursing pressure,Desire for professional team and auxiliary equipment. Conclusion In the process of promoting the early functional exercise for severe patients,we should not only effectively use positive guidance,but also accelerate the construction of management specifications and operation guidelines,and study improvement strategies from multiple perspectives such as improving human resources and auxiliary equipment configuration,so as to improve the prevention and treatment effects of ICU-AW in severe patients.

    Effect of perioperative transcutaneous electrical acupoint stimulation on postoperative gastrointestinal function in patients with colorectal cancer:a Meta-analysis

    Objective To investigate the effect of perioperative transcutaneous electrical acupoint stimulation on postoperative gastrointestinal function in patients with colorectal cancer by meta-analysis. Methods PubMed,Embase,the Cochrane Library,Web of Science,China Biomedical Literature Database,Wanfang Data Knowledge Service Platform,China National Knowledge Infrastructure,VIP database were electronically searched to collect randomized control trials on the effects of perioperative transcutaneous electrical acupoint stimulation on postoperative gastrointestinal function recovery in colorectal surgery patients from inception of database to June 2022. Two reviewers with evidence-based nursing training independently screened the literature according to the inclusion and exclusion criteria,extracted data,and evaluated the quality, and then performed meta-analysis by Stata14.0 software. Results A total of 14 literature were included,involving 1 560 patients.The results showed that transcutaneous electrical acupoint stimulation could accelerate the recovery of bowel sounds[WMD=-9.58,95%CI(-12.30,-6.88),Z=6.94,P<0.001] and shorten the time of anal exhaust[WMD=-12.35,95%CI(-15.58,-9.12),Z=7.49,P<0.001],reduce the incidence of nausea and vomiting[RR=0.46,95%CI(0.32,0.66),Z=4.18,P<0.001],accelerate the time of first defecation[WMD= -15.51,95%CI(-22.16,-8.86),Z=4.57,P<0.001],shorten hospitalization days[WMD=-1.64,95%CI(-2.75,-0.54),Z=2.92,P=0.004]. However,there was no significant difference in the incidence of intestinal obstruction[RR=0.62,95%CI(0.26,1.45),Z=1.11,P=0.27]. Conclusion Transcutaneous electrical acupoint stimulation can promote the recovery of gastrointestinal function in patients with colorectal cancer after surgery. However,there is no consensus on the selection of acupoints,intervention timing,frequency,and the selection of parameters of transcutaneous electrical acupoint stimulation instrument,which needs to be validated by further high-quality clinical studies.

    Barriers of early rehabilitation based on the view of ICU patients and family members:a qualitative meta-synthesis

    Objective To systematically evaluate and synthesize the barriers of early rehabilitation based on the view of ICU patients and family members. Methods PubMed,Web of Science,Embase,CINAHL,Cochrane Library,CNKI,Wanfang,VIP and SinoMed Database were searched for qualitative studies on early rehabilitation involving ICU patients and their family members from the establishment of database to June,2022. The quality of included literature were assessed using Australian JBI Evidence-Based Health Care Center(2017 edition)Quality Evaluation Standards and the results were integrated by an aggregative approach. Results A total of 13 studies were included,48 findings were refined,7 new categories were formed,and 3 integrated results were obtained. The first integrated result was inadequate understanding of early rehabilitation concepts,knowledge and identity responsibilities among ICU patients and family members. The second integration result was the heavy physical and mental burden of early rehabilitation and the third one was unmet multiple needs of patients and family members. Conclusion There are many barriers of early rehabilitation involving ICU patients and family members. Managers and clinical staff should take measures to address barriers and promote the clinical implementation of early rehabilitation in ICU actively.

    To summarize the rehabilitation nursing experience of a child with catecholamine sensitive polymorphic ventricular tachycardia(CPVT)who suffered from secondary brain injury after successful cardiopulmonary resuscitation. CPVT is easy to be induced by exercise or emotional excitement. Attention was paid to pacify children and create a quiet ward environment. Progressive exercise and cognitive language rehabilitation was carried out under continuous ECG monitoring,and individualized nutrition management was performed to improve the quality of life of the child. After 45 days of rehabilitation nursing,the children's consciousness,language cognition and motor function were significantly improved. During the hospitalization,there was no recurrence of CPVT. He was stable and discharged.

    Objective To form a consensus of nursing experts on extracorporeal membrane oxygen support therapy for children (hereafter referred to as the “consensus”),regulating the care of children during the period of extracorporeal membrane oxygen support therapy. Methods Based on the systematic search of domestic and foreign literature,combined with the results of interviews with stakeholders,specific problems were listed and the expert letter consultation form was designed. 15 experts in the relevant fields from Beijing,Shanghai,Chongqing and Zhejiang were invited to conduct two rounds of Delphi expert letter consultation and one round of expert demonstration meeting,so as to adjust,modify and improve each item. Results The consensus included 10 aspects as following:ECMO preparation before establishment,selection of ECMO support mode,parameter setting and system monitoring,circulation management,respiratory management,drug management,nutrition management,complication management,transport management and discharge follow-up management. Conclusion This consensus is scientific and practical,and provides guidance for the nursing practice of ECMO support therapy in children.

    To summarize the nursing experience of a case of pregnancy complicated with Taussing-Bing syndrome. For the serious and difficult problems of maternal and child monitoring and complication prevention in perinatal period,nursing measures such as cardiopulmonary support nursing during perinatal period,fetal intrauterine status monitoring during pregnancy,prevention of perinatal complications were adopted. After 7 days of careful treatment and nursing,the patient's condition was stable and discharged. After discharge,the patient was followed up for 3 months,and the mother and son were in good condition.

    To summarize the nursing experience of a child with congenital generalized lipodystrophy accompanied by hypovolemic shock. The main points of nursing care included closely observing the changes of the condition,rapid identification of high-risk events using the Pediatric Early Warning Score,early identification of hypovolemic shock,and timely target-oriented fluid management intervention,providing individual enteral nutrition support and family health guidance according to the characteristics of the disease and disease course. After 16 days of treatment and nursing,the child was discharged from hospital. No serious complications occurred during the follow-up for more than 1 year.

    To report the preoperative nursing for a case of paraquat poisoning patient undergoing lung transplantation under long-term ECMO support. In terms of the patient's acute respiratory distress syndrome,multi-organ dysfunction syndrome and respiratory failure after paraquat poisoning,multiple targeted nursing measures were taken,including the formation of an ECMO multidisciplinary team,continuous renal replacement therapy,control of pulmonary infection,monitoring and nursing of ECMO complications,and strengthening psychological care. After 35 days of ECMO treatment,the patient successfully received lung transplantation and was good during postoperative follow-up.

    To summarize the experience of postoperative nursing of a child with congenital bilateral posterior nasal stenosis who underwent nasal endoscopic bilateral nasal stenosis dilation. Aiming at the problems of prolonged indwelling time of dilation tubes,risk of malnutrition,and high incidence of restenosis within one year after surgery in the child,the importance of maintenance of nasal dilation tubes and prevention of complications were emphasized,including safe fixation,effective removal of secretions,the maintain of skin integrity. An individualized nutritional support plan was formulated and extended services for home care and follow-up after discharge were implemented. After 51 days of careful treatment and care,the child was improved and discharged. And he was in good condition during the four follow-ups after discharge.

    This paper summarized the nursing experience of 5 patients with fulminant type 1 diabetic ketoacidosis caused by immune checkpoint inhibitors. In view of the characteristics of acute onset and critical condition of fulminant type 1 diabetic ketoacidosis caused by immune checkpoint inhibitors,we adopted fluid rehydration,correction of water,electrolyte and acid-base balance,standardized insulin therapy and use of insulin pump,blood glucose monitoring during hospitalization and application of immune checkpoint inhibitors,continuous care after discharge and early identification of ketoacidosis. After careful treatment and other nursing measures,5 patients had stable blood glucose control,were recovered and discharged. Regular follow-up showed no recurrence of related complications.

In view of the complex problems of patients' medical conditions and postoperative complications,measures such as circulatory goal-oriented management,implementing individualized enzyme replacement therapy,observing postoperative complications,and providing personalized rehabilitation plan were taken. On postoperative day 25,the patient was discharged in a stable condition.

    Influencing factors and countermeasures of delayed rapid response team activation in Neurosurgery Department

    Objective To explore the influencing factors and countermeasures of delayed rapid response team (RRT)activation in the Department of Neurosurgery. Methods We retrospectively analyzed the data of hospitalized patients treated with RRT from the Department of Neurosurgery in a tertiary class A hospital in Hangzhou,Zhejiang Province from January 2019 to June 2022. The patients were divided into two groups(delayed and non-delayed)according to whether the call duration was longer than 15 minutes. We compared the differences in clinical indicators between the two groups and analyzed influencing factors of delayed RRT activation. Results A total of 86 patients(59.3%)were found to experience delayed RRT activation among 145 included patients. Logistic regres-sion analysis showed that aged 65 and over,having more than two comorbidities,a score of GCS less than 9,and night duty(17:01—07:59)were independent risk factors for delayed RRT activation(P<0.05). A score of MEWS(Modified Early Warning Score)more than 7 was an independent protective factor(P<0.05). Conclusion Nurses should pay attention to aged or critically ill patients,patients with multiple comorbidities or nursed during night duty and ensure the continuity of care. It is suggested to establish a specialized early warning system for neurosurgery patients. The hospitals should strengthen monitoring and data analysis and provide training for nurses through diversified ways to help them summon the rapid response system effectively.

    Meta-analysis of influencing factors of mild cognitive impairment in patients with traumatic brain injury

    Objective To identify the risk factors of mild cognitive impairment in patients with traumatic brain injury by meta-analysis. Methods China National Knowledge Network,Wanfang Database,China Biomedical Literature Database,PubMed,Embase,Cochrane Library and other databases were searched for studies on mild cognitive impairment in patients with craniocerebral injury from the establishment of the database to August 2022 and the references of included studies were included. Two reviewers screened the literature according to the inclusion and exclusion criteria,and extracted data,assessed the quality of included studies independently. Meta-analysis was conducted by Rev Man 5.3. Results A total of 17 studies and 16 risk factors were included,with a sample size of 11 750 cases. The meta-analysis result showed that the risk factors were age(OR=3.03),education level(OR=2.46),damage of frontal lobe(OR=4.84),damage of temporal lobe(OR=6.01),damage of left hemisphere(OR=5.02),multi-site injury(OR=6.80). Conclusion Clinical nursing staff should strengthen the assessment and screening of cognitive impairment in patients with brain injury,pay attention to the potential impact of cognitive reserve(age and education level)and the characteristics of different damaged areas of cognitive domain,target early and effective intervention to improve the clinical outcome of patients.

    Summary of the best evidence for the management of early oral feeding in postoperative patients with esophageal cancer

    Objective To retrieve and summarize the relevant evidence of early oral feeding management in patients with esophageal cancer after surgery. Methods According to the evidence-based “6S” model to retrieve the evidence,we searched all the evidence about the management of early oral feeding in postoperative patients with esophageal cancer in UpToDate,BMJ Best Clinical Practice,Australia's Joanna Briggs Institute Evidence-based Practice Database,National Institute for Health and Care Excellence,CNKI,Wanfang Database,China Biomedical Literature Database and related websites or databases,including clinical decisions,guidelines,systematic reviews,meta-analysis,expert consensuses,evidence summaries,and randomized controlled trials. The retrieval time limit was from the establishment of the database to May 2022. The quality of the literature was evaluated independently by two researchers who had received systematic evidence-based nursing training,and the evidence meeting the quality standards was extracted and summarized based on the judgment of professionals. Results 14 literature were eventually included,including two clinical decisions,four guidelines,three expert consensuses,one systematic review and four randomized controlled trials. 24 pieces of best evidence were summarized from 4 aspects of early oral feeding:choice of timing,the evaluation of swallowing function,the formulation of eating plan,risk identification and prevention. Conclusion Early oral feeding is safe and feasible in postoperative patients with esophageal cancer. Nursing staff should select the best evidence based on clinical situation,facilitating factors and barriers of evidence application and patients' willingness,so as to provide scientific and evidence-based management programs for early oral feeding of patients after esophageal cancer surgery,which will accelerate postoperative recovery of patients,reduce complications and shorten hospital stay.

    Research progress of core outcome sets in the emergency and critical care patients

    There are various outcome indicators in clinical studies of acute and critical patients,which are difficult to integrate. The core outcome set(COS)refers to the standard set of clinical outcomes that should be measured and reported in clinical research. This article reviews the characteristics and content of developing COS. Multiple stakeholders such as physicians and nurses,patients and families should be involved during COS development. Outcomes such as physical function,neurological and psychological status,activity and participation,as well as society and environment should be involved. The construction of a COS for critically ill patients can provide a reference for standardizing nursing research design and promote the transformation and integration of nursing evidence,but the core index set needs to be further refined and improved.

    Pre-hospital emergency care refers to the field rescue,transportation and on-way rescue for all kinds of life-threatening emergencies,trauma,poisoning,disasters and accidents in the environment outside the hospital,that is,the rescue from the onset or injury of the patient to the hospital for medical treatment. This article reviews the current status of pre-hospital first aid training,and proposes a three-level linkage training mode for employees from general hospitals,medical alliances and expressways to provide comprehensive and convenient pre-hospital first aid training for highway workers.

    Objective To make an artificial anal bag and explore its application effect. Methods The artificial anal bag was composed of a chassis and a bag body which was equipped with a one-way exhaust valve. A total of 84 patients with artificial stoma after radical resection of colorectal cancer in a tertiary Grade-A hospital from December 2021 to May 2022 were selected as the application objects. The patients were divided into the experimental group(42 cases)and the control group(42 cases)by random number table method. The experimental group received self-made ventable artificial anal bags,and the control group received conventional artificial anal bag. The differences of postoperative defecation time,feeding time and length of hospital stay between the two groups were compared,as well as the satisfaction of medical staff and patients,the incidence of postoperative ileus between the two groups. Results The postoperative defecation time,feeding time and hospitalization days of experimental group were less than those of control group,the differences were statistically significant(P<0.05). The satisfaction of experimental group was higher than control group,the difference was statistically significant(P<0.001). The incidence of postoperative ileus in experimental group was significantly lower than that in control group(χ2=6.098,P<0.05). Conclusion The ventable artificial anal bag is convenient to operate,time and labor saving,improve the comfort and satisfaction of patients,reduce the incidence of intestinal obstruction,shorten the average length of hospital stay,and better implement the concept of enhanced rehabilitation.
