    Application of near miss management combined with standardized communication model in hospital transportation of critical patients in nephrology department

    Objective To evaluate the application effect of near miss management combined with situation-background-assessment-recommendation(SBAR)standardized communication model in hospital transportation of critical patients in nephrology department. Methods We selected the critically ill patients who need to be transferred to ICU in the department of nephrology of a tertiary class A general hospital as the research object by convenient sampling. 112 patients from January to June 2022 were selected as the test group and received near miss management combined with SBAR communication mode for in-hospital transfer. 103 patients from June to December 2021 were selected as the control group and received the conventional in-hospital transfer method. Compare the incidence of nursing near miss events,transfer time,and the score of nurses' knowledge,belief and practice on hospital transfer of critical patients between the two groups. Results The incidence of transport near miss events in the test group(18.75%)was lower than that in the control group(38.83%)(χ2=10.651,P<0.05). The length of transport preparation,handover time after transfer and total handover time of transportation in the test group were lower than those in the control group(P<0.001). But there was no significant difference in transport time(P=0.537). After training and clinical practice,the score of knowledge,belief and practice of nurses on hospital transfer of critical patients was(154.32±5.00),which was significantly higher than that before training(120.56±6.25)(t=10.734,P<0.05). Conclusion The implementation of near miss management in the transport of critically ill patients in the department of nephrology and the use of SBAR communication mode for the transfer will reduce the incidence of near miss events,shorten the transport and handover time,improve the efficiency and quality of transport.

    Development and psychometric testing of ICU Nurses' Cognition of Acute Skin Failure Questionnaire

    Objective To develop a questionnaire for ICU nurses' cognition of acute skin failure and test its reliability and validity. Methods Based on the knowledge,attitude and practice theory,literature review method and expert letter consultation method were used to formulate preliminary questionnaire of ICU Nurses' Cognition of Acute Skin Failure,and the questionnaire was adjusted and modified through pre-investigation. From July to August 2022,254 ICU nurses were conveniently selected from a tertiary class A hospital of Zhejiang Province as the survey objects. The questionnaire was used to evaluate the validity and reliability of the questionnaire. Results ICU Nurses' Cognition of Acute Skin Failure Questionnaire included 33 items in 3 dimensions. The S-CVI was 0.848,and the I-CVI was 0.889 to 1.000. Exploratory factor analysis extracted three common factors,and the cumulative variance contribution rate was 73.290%. The Cronbach's α coefficient of the total questionnaire was 0.941,and the test-retest reliability was 0.956. Conclusion ICU Nurses' Cognition of Acute Skin Failure Questionnaire has good reliability and validity,which can be used as a survey tool to evaluate the cognitive level of ICU nurses on acute skin failure.

    Study on the effect of progressive pulmonary rehabilitation program in patients with respiratory failure

    Objective To explore the implementation effect of the progressive pulmonary rehabilitation program on patients with respiratory failure. Methods A quasi-experimental research design was adopted. 48 patients with respiratory failure from January 2021 to September 2022 were selected as the intervention group and the progressive pulmonary rehabilitation program was adopted. 43 patients with respiratory failure from January 2019 to December 2020 were selected as the control group and routine pulmonary rehabilitation was implemented. Compare lung function index,mechanical ventilation time,ventilator weaning time,reintubation rate within 48 hours after weaning,and death outcome between the two groups. Results In the intervention group,the mechanical ventilation time(Z=-2.982,P=0.003)and ventilator weaning time(Z=-4.189,P<0.001)were significantly shortened,the reintubation rate within 48 hours after weaning was reduced(χ2=-4.501,P=0.034),the lung function improved(P<0.05),but there was no significant difference in death outcome(χ2=0.052,P=0.819). Conclusion The progressive pulmonary rehabilitation program can improve the lung function of patients,shorten the time of mechanical ventilation,and reduce the rate of reintubation after weaning.

    Objective To develop and implement prone positioning program for awake patients,and explore the feasibility and effeteness of prone positioning in awake patients. Methods The multidisciplinary treatment team was set up to develop the prone positioning program,including treatment decision of awake prone position timing,prone positioning schedule making with patients,prone position safety and tolerance assessment,position replacement scheme to improve tolerance,individualized time accumulation system,and prone position knowledge education and psychological support. The preliminary application was carried out in awake COVID-19 patients admitted to a designated hospital in Fujian province in September 2021. Results 136 patients were treated with awake prone positioning program,the average duration were (11.95±2.47)h. The prone positioning tolerability scores were significantly improved one week after hospital admission(t=35.254,P<0.05). 133 patients recovered and 3 patients(2.2%)developed to critical illness,the average of hospitalization length was (14.95±3.02)days without serious adverse events during prone positioning. The oxygenation index of patients were significantly improved(F=5.624,P<0.05). Conclusion The awake prone positioning program based on multidisciplinary treatment team cooperation is feasible and practical,which can provide a reference for the improvement of lung ventilation in clinical practice.

    A qualitative study of nurses' experience in pulmonary rehabilitation of primary Sjogren's syndrome-related interstitial lung disease

    Objective To explore the experience of nurses in providing pulmonary rehabilitation nursing for patients with primary Sjogren's syndrome(PSS)-related interstitial lung disease(ILD),so as to provide a basis for promoting high-quality pulmonary rehabilitation nursing. Methods From June 2020 to March 2021,the semi-structured interview method was used to conduct in-depth interviews with nurses in the respiratory and critical care medicine departments of two tertiary class A general hospitals in Yangzhou City,and the Colaizzi 7-step analysis method was used to collect,organize,analyze the data and extract the theme. Results Four major themes were analyzed:many pressures of nurses during pulmonary rehabilitation training and difficulty in pulmonary rehabilitation care;appreciation of the importance of pulmonary rehabilitation knowledge and strong willingness to learn about pulmonary rehabilitation;perceiving the needs of good preparation before pulmonary rehabilitation and improving of the whole pulmonary rehabilitation assessment;expecting multidisciplinary cooperation and obtaining support from various aspects such as managers and family members. Conclusion It is necessary to pay attention to the pressure and needs of nurses in the process of providing pulmonary rehabilitation nursing for patients with primary Sjogren's syndrome-related interstitial lung disease. While promoting the physical and mental health of nurses,we should strengthen the learning of pulmonary rehabilitation knowledge,optimize the pulmonary rehabilitation management model of sports and medical integration,and encourage family members to participate,so as to improve the nursing ability of nurses in pulmonary rehabilitation.

    Research progress on related factors and nursing care of micro-aspiration in ICU patients

    Micro-aspiration is caused by subglottic secretions entering the lower respiratory tract through a channel formed by the folds of the artificial airway balloon. It is considered to be one of the important pathogenic factors of ventilator-associated pneumonia. Because the patients do not show any symptoms after micro-aspiration and it is highly occult,the diagnosis will be delayed,which requires further exploration and confirmation. In this paper,the related factors and prevention and treatment measures of micro-aspiration were summarized,and the causal relationship between micro-aspiration and ventilator-associated pneumonia was expounded to facilitate medical staff to prevent and avoid the occurrence of micro-aspiration in patients in time,so as to reduce the incidence of ventilator-associated pneumonia.

    To summarize the nursing experience of lung transplantation in a child with bronchiolitis obliterans after bone marrow transplantation. Individualized children pulmonary rehabilitation nursing was taken for the patient's young age,complex disease,two transplants,insufficient engagement for cooperation,etc. That included game-style and reward-based respiratory exercises,modified prone ventilation,step-by-step exercise sequential nutritional support and psychological nursing. After 120 days of treatment and nursing,the patient was discharged from the hospital in a stable condition. The child recovered well during two months follow-up.

    To summarize the first aid and nursing experience of a child with severe acute respiratory distress syndrome after liver transplantation. In order to solve the problems of the child with rapidly progressive hypoxemia and easy occurrence of bleeding,thrombosis and withdrawal symptoms during treatment,following measures were taken,such as emergency establishment of artificial airway,rapid implementation of extracorporeal membrane oxygenation and prone ventilation therapy,precise anti-coagulation management,rational sedation and analgesia,and development of personalized psychological nursing plan. After 28 days of treatment and nursing,the patient was transferred from the transplantation ICU to the general ward and discharged from the hospital on the 55th day after surgery. The follow-up was 2 months and the patient was in good condition.

    To summarize the nursing experience of a patient with severe pulmonary hypertension undergoing lung transplantation under multimodal extracorporeal membrane oxygenation(ECMO). For the complications of ECMO treatment for severe pulmonary hypertension before lung transplantation,veno-artery-venous ECMO(VAV-ECMO)was used as adjuvant therapy. Key points of nursing included VAV-ECMO switching nursing,dynamic monitoring and nursing during operation,safety nursing of anticoagulation and bleeding,weaning nursing of VAV-ECMO. Among them,the coordination of ECMO multi-mode switching and the cardiopulmonary rehabilitation nursing oriented to control pulmonary artery pressure were the difficulties and the key points of nursing. The ECMO was successfully removed on the second day of lung transplantation,and the endotracheal tube was successfully removed on the fourth day after surgery. The patient was discharged from hospital in a stable condition 23 days after admission.

    To summarize the nursing experience of a child with acute hepatic encephalopathy caused by late-onset ornithine carbamoyltransferase deficiency. In view of the ineffectiveness of drug treatment in the child with persistent hyperammonemia,serious central nervous system damage,and high nutritional risk,emergency continuous blood purification combined with ammonia scavenger through bypass metabolism to reduce blood ammonia,closed monitoring of encephalopathy caused by excess ammonia accumulation,early warning and first aid of encephalopathy should be done for the patient;nutritional strategies were dynamically adjusted according to blood ammonia levels,especially protein and amino acid intake,and psychological support for child and parents was provided to enhance confidence to overcome the disease. After 28 days of treatment and nursing,the child was discharged in stable condition,and the condition was good during one month of follow-up.

    To summarize the nursing experience of a case of laryngeal edema secondary to severe poisoning of pinellia ternata. In view of the main problems such as recurrent laryngeal edema,dyspnea and asphyxia of this patient,multidisciplinary collaborative treatment,control of toxic laryngeal edema,ginger compatibility detoxification,modified gastric lavage detoxification,and staged psychological nursing were carried out. After 6 days of treatment and nursing,the patient's condition was stable and discharged from the hospital. During 3 months of follow-up after discharge,the patient was in good condition.

    To summarize the first aid and nursing experience of resuscitative aortic balloon occlusion for hemostasis in a patient with massive upper gastrointestinal hemorrhage. In order to solve the problems of this patient,such as large amount of bleeding,unknown location of bleeding,critical condition with rapid progress,and the need to quickly control bleeding to maintain vital signs,we took measures such as rapid disease identification and restricted fluid resuscitation,emergency bedside resuscitated aortic balloon occlusion for emergency hemostasis and close observation of ischemia and reperfusion injury. After 13 days of active treatment and meticulous nursing,the patient was successfully treated and discharged from the hospital. The patient was followed up for 3 months after discharge and was in good condition.

    To summarize the nursing experience of a child with Kawasaki disease shock syndrome complicated with medium coronary aneurysm and intracranial hemorrhage. We should targeted at the nursing difficulties,including the high mortality of Kawasaki disease shock syndrome,complicated coronary aneurysm which was easy to form thrombus leading to myocardial ischemia,infarction and even sudden death,and no specific clinical manifestation in the early stage of intracranial hemorrhage. Nursing points:medical cooperation to quickly correct Kawasaki disease shock syndrome;continuous dynamic observation of the child with vital signs and nervous system abnormal changes,effective identification of signs of intracranial hemorrhage and timely intervention;precise application of anticoagulant drugs to prevent complications caused by coronary aneurysm;developing individualized home care plan to promote positive follow-up. After 13 days of active treatment and intensive care,the patient's condition was controlled,and transferred to the Department of Cardiology for consolidation treatment,then he was discharged and in good condition during six months follow-up.

    Postoperative nursing of children with propionic acidemia undergoing parent-child liver transplantation

    The postoperative nursing experience of two children with propionic acidemia undergoing parent-child liver transplantation was summarized. In view of the problems of multiple organ involvements,metabolic disorders,vascular complications and difficulty of sedation in this group of children,the following interventions were adopted,including monitoring the function of important organs and reporting abnormal conditions timely,the maintain of acid-base balance and being cautious of acidosis and hyperammonemia,strict implementation of anticoagulation therapy to reduce vascular-related complications,effective sedative care to prevent the occurrence of convulsions,accurate administration of immunosuppressants to prevent acute rejection,special formulated diets and control of protein intake. After active treatment and meticulous care,two children were discharged from the hospital after surgery and followed up for more than 1 year with a good prognosis.

    To summarize the nursing experience of a case of Gerstmann-Sträussler-Scheinker syndrome with thyroid crisis. In view of nursing problems including rapid progress of disease,thyroid crisis,infection prevention and control,interventions were adopted,including recognizing the disease risk points and emphasis on preventing aspiration and falls,identifying nursing difficulties and key points through bedside three-level rounding,quick recognition of thyroid crisis and rescue,implementation of nosocomial infection prevention and control,and genetic risk notification. After 16 days of treatment and nursing,the patient was discharged from the hospital after his condition was stable. After discharge,she was followed up for 3 months and was in good condition.

    To summarize the nursing experience of a patient with diabetic ketoacidosis complicated with stress cardiomyopathy leading to acute circulatory failure and received extracorporeal membrane oxygenation combined with continuous renal replacement therapy after extracorporeal cardiopulmonary resuscitation. In view of the atypical symptoms of chest pain and the rapid progress of the disease at the onset of the disease,extracorporeal cardiopulmonary resuscitation should be initiated as soon as possible and nursing cooperation should be performed under the premise of maintaining the stability of the patient's internal environment,coagulation disorders was identified and nursing care was provided. For the patients with unstable circulation and contradictions of turning over,predictive prevention of pressure injuries was performed,and early rehabilitation exercises were implemented in patients with stable circulation in different disease stages. After 25 days of treatment and nursing,the patient was stable and discharged. After 2 months of follow-up,the patient was in good condition.

    To summarize the nursing care of 20 children with very early onset inflammatory bowel disease (IBD)during the active period. In response to the specific situations of patients,including the early onset of IBD,negative consequences of IBD on growth and development,the penetration of inflammatory lesions and the formation of fistulas,the difficulty of intestinal preparation when evaluating the drug response,and the high level of psychological pressure of family members due to life-long duration of IBD,the nursing measures were taken. Early nutritional management was adopted to improve the prognosis of the disease. Standardized application of biological agents and prevention of anaphylactic shock were conducted. Skin care was strengthened when fistula formed,intestinal preparation was improved to evaluate the treatment effects. The nurses also completed the health guidance and follow-up to improve treatment compliance. After active symptomatic treatment and nursing,all the children were improved and discharged.

    Investigation of the status and influencing factors of frailty in emergency elderly patients

    Objective To investigate the status of frailty in emergency elderly patients and analyze the influencing factors. Methods Using the continuous sampling method,we selected 254 elderly patients hospitalized in the emergency department of a tertiary class A hospital in Shanghai from November 2021 to February 2022 as the research subjects,and divided them into frailty group and non-frailty group. The general data questionnaire and Chinese Geriatric Frailty Screening Scale were used for survey and data collection. Results Frailty occurred in 72 emergency elderly inpatients(28.3%). The scores of frailty were significantly different among patients at different ages, with different health status, respiratory function, multiple medication,multiple comorbidity,hemoglobin,BMI,hospitalization in the past year(P<0.05). Logistic regression showed that BMI≥24(OR=0.422)was a protective factor for frailty in elderly emergency patients,poor health(OR=8.951),multiple comorbidities(OR=3.983),and low hemoglobin(OR=2.083)were risk factors for frailty(all P<0.05). Conclusion The incidence of elderly frailty is high,and it is necessary to establish and improve frailty assessment and intervention mechanism for elderly inpatients in emergency department.

    Best evidence summary on discharge follow-up of patients with aortic dissection

    Objective To retrieve,evaluate and summarize the best evidence of discharge follow-up for patients with aortic dissection. Methods We searched BMJ Best Practice,UpToDate,National Guideline Clearinghouse,Scottish Intercollegiate Guidelines Network,National Institute for Health and Care Excellence,Guidelines International Network,New Zealand Guidelines Group,Medlive,Registered Nurses' Association of Ontario,Joanna Briggs Institute library,Google,Cochrane library,Embase,PubMed,CINAHL,China Biology Medicine disc,CNKI,WANGFANG,and professional societies such as International Registry of Acute Aortic Dissection,Asian Society for Cardiovascular Surgery,American College of Cardiology,American Heart Association,The European Society of Cardiology,to collect relevant evidence,including clinical decision,guidelines,evidence summaries,expert consensuses and systematic reviews published from database construction to January 24th of 2022. Two researchers evaluated the quality of the literature,and extracted the evidence. Results Totally 15 articles were included,including 4 clinical decisions, 3 guidelines, 7 expert consensus and 1 systematic review. Finally,14 pieces of best evidence about principles of follow-up,follow-up time/frequency,imaging examination guidance,drug guidance,blood pressure guidance,lifestyle guidance,pregnancy guidance and complication management were summarized. Conclusion This study summarized the best evidence of discharge follow-up for patients with aortic dissection. Medical staff can develop effective and feasible nursing strategies according to patients' wishes and clinical conditions and professional judgment while applying evidence,so as to implement scientific follow-up management for patients and improve the quality of care.

    The article reviewed the clinical characteristics and nursing progress of infection in recipients of liver transplantation from organ donation after citizens' death(DCD). It was proposed that the infection after DCD liver transplant not only had complex pathogens,but also had a high early infection rate and diverse infection sites. In addition to the traditional predisposing factors,some new factors need to be paid attention to,such as donor-derived infection and marginal donor liver. Although organ donation has become the most important way of obtaining donor livers in China,currently there is a lack of nursing standards for infection after DCD liver transplantation in China,which needs to be further explored.

    A review of stress exposure management in premature infants during hospitalization

    Stress exposure of premature infants means that they are exposed to different stressors. This article reviewed the overview of stress exposure of premature infants,the control of the cumulative amount and the interventions of stress exposure of hospitalized premature infants. The control of the cumulative amount of stress exposure mainly included the control of the physical environment,the reduce of unnecessary medical treatment,testing and nursing procedures,shortening of the length of hospital stay,and adopting two-person techniques. Stress exposure intervention can reduce the negative effects of stress exposure on premature infants,but attention should be paid to the implementation rate of intervention measures.

    Objective To design a positioning and quantitative alarm device and explore its application effects in continuous drainage of lumbar cistern. Methods The positioning and quantitative alarm device was composed of lifting adjusting rod,alarm device,laser pointer,scale,lumbar cistern drainage bag,side panel,main panel,and etc. 21 bed ridden patients with lumbar cistern drainage tube were selected from the critical care department of a tertiary class A hospital in Hangzhou from May 2022 to September 2022 as the experimental group and were positioned with drainage tube positioning and quantitative alarm device. A total of 21 bed ridden patients with lumbar cistern drainage tube were selected from January 2022 to April 2022 as the control group,and were positioned with traditional tools. The average total positioning time,difference of average height measurement data and nursing staff satisfaction were compared between the two groups. Results The average total measuring time of lumbar cistern drainage bag in experimental group was (12.32±2.93)s,which was shorter than that in control group (33.24±8.84)s,and the difference was sta-tistically significant(t=38.298,P<0.001). The difference of average height measurement data in the experimental group was (1.25±0.21)mm,which was less than that of the control group (7.03±1.92)mm,and the difference was statistically significant(t=20.456,P<0.001). The satisfaction of nursing staff in experimental group(100%)was higher than that in control group(28.57%),and the difference was statistically significant(P<0.05). The daily drainage volume in the experimental group was(162±13)ml,its target accuracy was higher than that of the control group[(165±37)ml]. Conclusion The application of positioning and quantitative alarm device in the continuous drainage of lumbar cistern can effectively save the time of positioning measurement,improve the satisfaction of nursing staff.
