高压力情境密室逃脱训练在急诊体外膜肺氧合护理团队能力提升中的应用

摘要:

目的 探讨基于高压力情境设计的密室逃脱训练在急诊体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)护理团队核心能力提升中的应用效果。方法 于2025年3月—6月使用便利抽样法选取杭州市某三级甲等医院急诊科参与ECMO救治的护士共64名,按层级采用随机数字表法分为对照组与试验组,各32名。试验组在常规培训基础上引入基于真实急诊ECMO流程构建的密室逃脱训练,对照组采用传统ECMO团队培训模式。通过理论知识考核、技能操作考核,并采用团队合作能力问卷及批判性思维能力量表对训练效果进行评价。结果 教学后,试验组ECMO理论知识、技能操作成绩均高于对照组(P<0.05);在团队合作能力及批判性思维能力各维度评分中,试验组均显著优于对照组(P<0.05)。结论 基于急诊ECMO高压力、时间敏感特征构建的密室逃脱训练,有助于提升护理团队在复杂急救环境下的综合能力,尤其在团队协作与批判性思维等非技术能力培养方面具有优势,可为急危重症护理团队培训提供新的实践路径。

Abstract:

Objective To explore the application effect of high-stress scenario-based escape room training on the core competency improvement of emergency extracorporeal membrane oxygenation(ECMO) nursing teams. Methods From March to June 2025,convenience sampling was used to recruit 64 nurses involved in ECMO resuscitation in the emergency department of a tertiary grade-A hospital in Hangzhou. They were randomly divided into an experimental group(n=32) and a control group(n=32) using a random number table. The experimental group received escape room scenario training built on real emergency ECMO procedures in addition to routine training,while the control group received conventional ECMO team training. The training effect was evaluated through theoretical knowledge tests,skill operation assessments,the Teamwork Ability Questionnaire,and the Critical Thinking Disposition Inventory. Results After the training,the experimental group scored significantly higher than the control group in ECMO theoretical knowledge and skill operation(P<0.05). The experimental group also demonstrated significantly better scores in all dimensions of teamwork ability and critical thinking ability compared to the control group(P<0.05). Conclusion Escape room scenario training designed based on the high-stress,time-sensitive features of emergency ECMO can enhance the comprehensive competence of nursing teams in complex resuscitation environments. It shows particular advantages in cultivating non-technical skills such as teamwork and critical thinking,and may provide a new practical pathway for the training of critical care nursing teams.
