脑卒中吞咽障碍信息化管理平台的构建及应用研究

Construction and application of information management platform for stroke dysphagia

摘要:

目的 构建脑卒中吞咽障碍信息化管理平台,并探讨其应用效果。方法 基于循证证据结合临床实际设置吞咽功能筛查、吞咽障碍评估、多学科协同干预的信息化智能提醒,通过改良Gugging吞咽功能评估量表(Gugging Swallowing Screen,GUSS)推送结构化护理措施,构建“筛查-评估-干预”三位一体的脑卒中吞咽障碍信息化管理平台。比较脑卒中吞咽障碍信息化管理平台应用前2024年10月—2025年2月和应用后2025年3月—7月常州市某三级甲等医院神经外科吞咽障碍复评漏评率、吞咽功能改善率、吞咽功能改善平均天数、护士吞咽障碍管理得分、护士满意度和患者出院时营养指标。结果 应用脑卒中吞咽障碍信息化管理平台后,吞咽障碍复评漏评率由36.4%降至0,差异有统计学意义(P<0.001);吞咽功能改善平均天数由9.00(7.25,11.00) d降至6.00(6.00,9.00) d,差异有统计学意义(P=0.002);试验组吞咽功能改善率、患者出院时营养指标高于对照组,护士吞咽障碍管理得分、护士满意度高于对照组,差异有统计学意义(均P<0.05)。结论 该研究构建的脑卒中吞咽障碍信息化管理平台,在单中心应用中显示出良好的效果,可规范管理流程、改善患者预后并提升护士专业能力。

Abstract:

Objective To construct an information management platform for stroke dysphagia and to explore its application effect. Methods Based on evidence combined with clinical practice,the information intelligent reminders of swallowing function screening,swallowing disorder assessment,and multidisciplinary collaborative intervention were set up,and structured nursing measures were pushed through the modified Gugging Swallowing Screen(GUSS),and a trinity of “screening-assessment-intervention” information management platform for stroke dysphagia was constructed. The missed rate,swallowing function improvement rate,average number of days of swallowing function improvement,nurse dysphagia management score,nurse satisfaction and nutritional indicators at discharge from October 2024 to February 2025 before the application of the stroke dysphagia information management platform and from March to July 2025 after application of the platform were compared in a tertiary A hospital in Changzhou. Results After applying the stroke dysphagia information management platform,the missed rate of swallowing disorder re-evaluation decreased from 36.4% to 0,with a statistically significant difference(P<0.001).The average number of days of swallowing function improvement decreased from 9.00(7.25,11.00) days to 6.00(6.00,9.00) days,and the difference was statistically significant(P=0.002). The improvement rate of swallowing function and nutritional indexes at discharge in the experimental group were higher than those in the control group. The management score of nurses and nurses’ satisfaction were higher than those of the control group,and the differences were statistically significant(all P<0.05). Conclusion The information management platform for stroke dysphagia constructed in this study has shown good results in single-center application,which can standardize the management process,improve patient prognosis and enhance the professional ability of nurses.
