二级库房信息系统在ICU一次性耗材精益管理中的应用研究

摘要:

目的 探讨二级库房信息系统联合精益5S管理模式在ICU一次性耗材管理中的应用价值,解决传统管理中存在的有效期监管盲区、盘点效率低下、耗材错漏收费及浪费严重等问题,提升管理精准度与护理安全。方法 构建二级库房信息系统联合精益5S管理模式,ICU二级库对接医院信息系统,耗材的二级库信息化管理包括耗材的入库、出库、库存预警、有效期管理等功能,确保耗材信息实时更新与共享,同时结合整理、整顿、清扫、清洁、素养5个方面,对ICU一次性耗材进行精细化管理,优化耗材存储环境,规范耗材使用流程。采用非同期对照研究,选取2024年1月—2025年6月某三级甲等医院ICU 84类一次性低值耗材及参与耗材管理的38名ICU护理人员为研究对象。2024年1月—9月为对照阶段,采用常规人工管理;2024年10月—2025年6月为试验阶段,实施二级库房信息系统联合精益5S管理。分析项目实施前后ICU二级库房管理的84类一次性低值耗材的管理效率和质量,分析单次盘点耗时、申领响应时间、耗材过期率、收费错漏率、不良事件率、人均日耗材成本和月度库存周转率等指标的变化情况。结果 联合管理模式实施后,管理效率显著提升,单次盘点耗时由干预前的(38.52±6.73) min降至(12.18±3.25) min(P<0.001),耗材申领响应时间从(4.21±1.05) h缩短至(1.05±0.33) h(P<0.001);耗材管理的质量得到改善,耗材过期率由5.37%降至0.76%(P<0.001),收费错漏率由3.12%降至0.53%(P<0.001),耗材不良事件率由1.57%降至0.35%(P<0.001);经济效益提升,人均日耗材成本由原来的(18.75 ± 3.62)元降至(9.83 ± 2.17)元(P<0.001),月度库存周转率由62.1%提升至89.6%。结论 二级库房信息系统在ICU一次性耗材精益管理中具有较好的应用效果,通过信息系统动态追踪与5S现场精细化管理,有效解决传统模式中的有效期失控、人工误差及资源浪费问题,提升管理效率,保障耗材安全,并实现成本精细化控制,为ICU耗材管理提供标准化路径。

Abstract:

Objective To evaluate the effectiveness of integrating a secondary storage information system with the Lean 5S Management Model in managing disposable medical supplies in the Intensive Care Unit(ICU),to address issues commonly associated with traditional management approaches,such as blind spots in shelf-life monitoring,low inventory efficiency,billing errors,and excessive waste,thereby improving management accuracy and patient safety. Methods A combined model of the secondary storage information system and lean 5S management was implemented in the ICU. The secondary storage system was interfaced with the hospital’s Health Information System(HIS),enabling digital management of consumables including inbound and outbound tracking,inventory alerts,and expiration monitoring. Real-time updates and information sharing were ensured. Simultaneously,the lean 5S principles—Sort,Set in order,Shine,Standardize,and Sustain—were applied to optimize storage conditions and standardize supply usage processes. A non-concurrent controlled study was conducted in the intensive care unit (ICU) of a tertiary Grade A hospital. Eighty-four categories of low-value disposable medical supplies used between January 2024 and June 2025 served as the study material. The control phase(January-September 2024) adopted conventional manual management,whereas the intervention phase(October 2024-June 2025) implemented a secondary storage information system combined with Lean 5S management. Key indicators included time per inventory count,response time for supply requisitions,expiration rate,billing error rate,adverse event rate,per capita daily supply cost,and monthly inventory turnover rate. Results After implementing the integrated model,management efficiency improved significantly. The average time for a single inventory count decreased from(38.52±6.73)minutes to(12.18±3.25)minutes(P<0.001),and the supply requisition response time dropped from(4.21±1.05) hours to(1.05±0.33) hours(P<0.001). Quality indicators also improved markedly,the expiration rate declined from 5.37% to 0.76%(P<0.01),the billing error rate from 3.12% to 0.53%(P<0.01),and the adverse event rate from 1.57% to 0.35%(P<0.001). In terms of economic outcomes,the per capita daily supply cost was reduced from(18.75±3.62)CNY to(9.83±2.17) CNY(P<0.001),while the monthly inventory turnover rate increased from 62.1% to 89.6%. Conclusion The integration of the ICU secondary storage information system with the Lean 5S Management Model demonstrates significant effectiveness in managing disposable medical supplies. Through real-time digital tracking and meticulous on-site management,this model effectively addresses issues of expiration oversight,human error,and resource waste inherent in traditional approaches. It enhances operational efficiency,ensures supply safety,and enables refined cost control,offering a standardized and scalable solution for ICU supply management.
