ICU护士操作性疼痛管理知信行问卷的编制及信效度检验

摘要：

目的编制ICU护士操作性疼痛管理知信行问卷并检验其信效度，为评估ICU护士操作性疼痛管理知信行提供可靠的研究工具。方法基于知信行理论，通过文献分析、专家函询形成ICU护士操作性疼痛管理知信行初始调查问卷。采用便利抽样法，于2025年7月—8月，选取浙江省5所三级甲等医院的800名ICU护士开展调查，对量表进行信效度检验。结果发放问卷800份，回收有效问卷787份，有效问卷回收率为98.38%。最终形成的问卷包括知识、态度、行为3个维度，共36个条目。2轮函询专家权威系数分别为0.88、0.90，肯德尔和谐系数为0.280、0.450，积极系数均为100%。问卷的内容效度为0.875~1.000，平均内容效度为0.901。因子分析表明问卷结构良好，累计方差贡献率为72.56%，验证性因子分析模型拟合佳。问卷总的Cronbach’s α系数为0.972，分半信度为0.843，重测信度为0.915。结论该研究编制的ICU护士操作性疼痛管理知信行问卷具有良好的信度与效度，可作为ICU护士操作性疼痛管理知信行水平的测评工具。

Abstract:

Objective To develop a questionnaire on knowledge，attitude，and practice（KAP） regarding procedural pain management among ICU nurses and examine its reliability and validity，so as to provide a reliable tool for assessing the KAP levels in this area. Methods Based on the KAP theoretical framework，the initial questionnaire was developed through literature review and Delphi expert consultation. A convenience sampling method was used to select 800 clinical nurses from five tertiary hospitals in Zhejiang Province for a survey conducted from July to August 2025，and reliability and validity tests were performed. Results A total of 800 questionnaires were distributed，yielding 787 valid responses，for an effective response rate of 98.38%. The final questionnaire included 36 items across knowledge，attitude，and practice dimensions. The expert authority coefficients for the two Delphi rounds were 0.88 and 0.90，with Kendall’s W of 0.280 and 0.450，and positive coefficients of 100% for both rounds. The content validity index ranged from 0.875 to 1.000，with an average of 0.901. Factor analysis indicated good structural validity，with a cumulative variance contribution rate of 72.56% and a good model fit in the confirmatory factor analysis. The Cronbach’s α of the questionnaire was 0.972 for the overall scale，split-half reliability was 0.843，and test-retest reliability was 0.915. Conclusion The KAP Questionnaire about procedural pain management exhibits good reliability and validity，and can serve as a reliable instrument for evaluating ICU nurses’ knowledge，attitudes，and practices in procedural pain management.
