川崎病休克综合征合并冠状动脉瘤及颅内出血患儿的护理

Nursing care of a child with Kawasaki disease shock syndrome complicated with coronary aneurysm and intracranial hemorrhage

摘要: 总结1例川崎病休克综合征合并中型冠状动脉瘤及颅内出血患儿的护理体会。针对川崎病休克综合征病死率高,并发冠状动脉瘤易形成血栓导致心肌缺血、梗死甚至猝死,并发颅内出血早期无特异性临床表现等难点,护理要点:迅速纠正川崎病休克综合征;关注神经系统等症状,早期识别颅内出血征象并及时干预;精准应用抗凝药物,预防冠状动脉瘤可能导致的并发症;制订个体化家庭护理方案,促进积极随访。经过13 d的积极治疗与精心护理,患儿病情得到控制,转至心内科治疗后好转出院,随访6个月状态良好。

Abstract: To summarize the nursing experience of a child with Kawasaki disease shock syndrome complicated with medium coronary aneurysm and intracranial hemorrhage. We should targeted at the nursing difficulties,including the high mortality of Kawasaki disease shock syndrome,complicated coronary aneurysm which was easy to form thrombus leading to myocardial ischemia,infarction and even sudden death,and no specific clinical manifestation in the early stage of intracranial hemorrhage. Nursing points:medical cooperation to quickly correct Kawasaki disease shock syndrome;continuous dynamic observation of the child with vital signs and nervous system abnormal changes,effective identification of signs of intracranial hemorrhage and timely intervention;precise application of anticoagulant drugs to prevent complications caused by coronary aneurysm;developing individualized home care plan to promote positive follow-up. After 13 days of active treatment and intensive care,the patient's condition was controlled,and transferred to the Department of Cardiology for consolidation treatment,then he was discharged and in good condition during six months follow-up.
