    Construction and effect research of early progressive resistance exercise program for the non-hemiplegic side in ischemic stroke patients

    Objective To construct the early progressive resistance exercise program for the non-hemiplegic side in ischemic stroke patients and evaluate its effects. Methods Through literature research and expert consultation,the progressive resistance exercise program of non-hemiplegic side in the early stage of ischemic stroke patients was constructed. A total of 70 patients with ischemic stroke hemiplegia who were hospitalized in two wards of the Department of Neurology from December 2023 to August 2024 in a Grade-III first-class hospital in Hunan Province were selected. There were 35 cases in each group according to the coin toss method. The control group received routine rehabilitation training,and the experimental group received early progressive resistance exercise intervention on the non-hemiplegic side on the basis of interventions in the control group. Balance ability,self-care ability,walking function,bilateral grip strength,lower limb muscle strength,muscle index and bone density were compared between the two groups.Results After intervention,the balance ability score of experimental group was higher than that of control group(Z=-3.060,P=0.002). Self-care ability,walking function,bilateral grip strength,muscle index and bone density were higher than those of control group(P<0.05),and the time of 5 sit-stand tests was shorter than that of control group(P<0.05). Conclusion The early progressive resistance exercise for the non-hemiplegic side is beneficial to improve balance,mobility,muscle strength,muscle index and bone density in hemiplegic patients with ischemic stroke.

    Research on the identification and classification of supportive care needs attributes of postoperative patients with liver cancer based on the Kano model

    Objective To analyze the postoperative supportive care needs of liver cancer patients using the Kano model,providing a basis for formulating supportive care service measures for postoperative patients with liver cancer.Methods The needs of supportive care were examined in 326 postoperative liver cancer patients from a tertiary hospital in Zhejiang Province from February 2023 to February 2024 using a self-designed questionnaire based on the Kano model. Results Using the Kano model to conduct needs analysis,the supportive care needs of postoperative patients with liver cancer included 9 attractive attributes,12 expected attributes,2 must-be attributes,3 indifferent attributes,and no reverse attributes were found. Conclusion Medical personnel can formulate useful health guidelines by using the Kano model to analyze supportive care for postoperative patients with liver cancer. It is of great significance to improve the quality of life of patients by dynamically identifying patients’ needs and providing supportive care services that meet their needs.

    Objective To explore the influencing factors of ICU nurses maintaining the dignity of elderly patients, to construct a conceptual framework and provide evidence for improving nursing practice. Methods A programmatic grounded theory approach was adopted. From March to May 2024,semi-structured interviews were conducted with 15 ICU nurses from three tertiary class A hospitals in Changsha. Data were analyzed using Nvivo software through three levels of coding. Results A total of 29 initial concepts were extracted,forming 9 axial codes,and ultimately 4 core categories were summarized,including nurse factors,patient factors,cultural factors,and management factors. A conceptual framework was constructed based on the intrinsic logical relationships among these categories. Conclusion Maintaining elderly patient dignity in the ICU requires strengthening nurse training,optimizing resource allocation,promoting multidisciplinary collaboration,and enhancing cultural sensitivity to establish a dignity-centered care environment.

    Brain protection after cardiopulmonary resuscitation(CPR) is a key intervention strategy to improve the prognosis of neurological function of patients,but its clinical nursing practice still faces the challenge of insufficient standardization. By systematically reviewing domestic and foreign guidelines,consensus and evidence-based evidence,this paper integrates the core strategies of brain protection nursing after CPR from the dimensions of multimodal assessment of neurological function,targeted temperature management(TTM),hemodynamic optimization,hyperbaric oxygen awakening and digital intelligence rehabilitation technology. Studies have proposed that the hierarchical intervention model based on dynamic monitoring and multidisciplinary collaboration mechanism can significantly improve clinical nursing efficacy. In the future,it is necessary to further construct standardized nursing pathways and explore the application of artificial intelligence in neurological prognosis prediction,so as to promote the precise development of brain protection nursing after CPR.

    Objective To explore the application effect of a continuous intra-abdominal pressure monitoring and warning system in enteral nutrition for prone mechanical ventilation patients.Methods The research enlisted 80 patients who were subjected to prone position mechanical ventilation in the Intensive Care Unit of a tertiary hospital in Shanghai from July 2022 to May 2024. Using the Random number table method,they were divided into intervention group and control group. The intervention group used the continuous intra-abdominal pressure monitoring and warning system in enteral nutrition and the control group used the conventional enteral nutrition treatment. The feeding compliance rates,the incidence of feeding intolerance symptoms within 7 days and nutritional indicators were compared between the two groups. Results The feeding compliance rate in intervention group was higher than the control group(86.75% VS 55.48%,P<0.001). The incidence of feeding intolerance symptoms in intervention group was lower than the control group(37.84% VS 64.86%,P=0.020). On the 8th day,the nutritional indicators in intervention group had higher levels of pre-albumin and albumin than the control group,the differences were statistically significant(P<0.05). The difference in calf circumference between the two groups was not statistically significant(P>0.05). Conclusion The application of continuous intra-abdominal pressure monitoring and warning system can help to improve the rate of feeding compliance,reduce the incidence of feeding intolerance,and enhance the indicators of nutrition. It has clinical guidance significance for the implementation of enteral nutrition in prone mechanical ventilation patients.

    Development of audit indicators and analysis of barriers for early enteral nutrition support in prone mechanical ventilation patients

    Objective To comprehensively evaluate the current status of clinical application of evidence for early enteral nutrition support in ICU prone mechanically ventilated patients and to develop audit indicators,and to analyze the barriers and facilitators,and to formulate strategies for change. Methods Through the evidence summary,16 items of best evidence were included,and 20 audit indicators were developed. Based on the results of the baseline audit of clinical practice,the barriers and facilitators were analyzed,and change strategies were developed. Results Among the 20 indicators reviewed in this study,the clinical implementation rate of only one indicator was 100%,15 indicators were 0,and the range of implementation rates for the other indicators was 8.7%~95.7%. The main barriers were the lack of standardized management procedures,lack of relevant knowledge and training for medical staff,lack of multidisciplinary cooperation. While facilitators were mainly strong leadership of managers,hospitals with rich experience in evidence-based practice and a mature team of specialist nurses,and high level of information technology. Conclusion There is a large gap between the best evidence for early enteral nutrition support for ICU mechanically ventilated patients in the prone position and clinical practice. We should fully utilize the facilitators,overcome the barriers,and implement strategies for change,to promote effective translation of evidence to clinical practice.

    Development and validation of a Knowledge-Attitude-Practice Questionnaire on Enteral Nutrition Care for Patients Undergoing Prone Position Ventilation

    Objective To develop a Knowledge-Attitude-Practice(KAP) Questionnaire on Enteral Nutrition Care for Patients Undergoing Prone Position Ventilation(PPV) and to evaluate its reliability and validity. Methods Based on the KAP theory,an initial pool of items was created through literature review and semi-structured interviews. The initial version of the questionnaire was finalized after expert consultation and a pilot study. From March to June 2024,a convenience sampling method was used to select ICU nurses from three hospitals in Zhejiang Province for survey. The reliability,item analysis,exploratory factor analysis(EFA),and confirmatory factor analysis(CFA) of the questionnaire were conducted. Results The final questionnaire consisted of three dimensions,including knowledge,attitude,and practice,with a total of 43 items. The overall Cronbach’s α coefficient of the questionnaire was 0.957,indicating high internal consistency. The cumulative variance contribution rate from the EFA was 72.128%. The indices of CFA showed good model fit(CFI=0.952,RMSEA=0.054). The content validity index was 0.978 and the content validity index ranged from 0.820 to 1.000,suggesting that the questionnaire has good content validity. Conclusion The developed questionnaire demonstrated good reliability and validity,making it a reliable tool for assessing ICU nurses’ knowledge,attitudes,and practices regarding enteral nutrition care for PPV patients.

    Experience of patients ventilated in awake prone positioning:a qualitative Meta-synthesis

    Objective To systematically evaluate the experience of patients maintaining awake prone positioning ventilation,and to provide a reference for the development of targeted intervention programs. Methods We searched CNKI,Wanfang Data,VIP database,China Biomedical Literature Database,PubMed,Embase,The Cochrane Library,Web of Science,PsycNET and CINAHL to screen for qualitative studies on experience of patients who ventilated in awake prone positioning,the retrieval time was from the establishment of the database to September 30,2024. The quality of the literature was evaluated using the quality evaluation criteria for qualitative research of the Joanna Briggs Institute(JBI) Evidence-based Health Care Center in Australia,and the results of the literature were integrated using the pooled integration method in the meta synthesis of the JBI Evidence-based Health Care Center in Australia. Results A total of six studies were included,and 64 themes were distilled and grouped into 13 categories,resulting in five integrated results,including perceived benefits of awake prone ventilation therapy,perceived physical and psychological challenges of awake prone ventilation therapy,barriers affecting the implementation of awake prone ventilation therapy,coping strategies to facilitate the implementation of awake prone ventilation therapy,and internal expectations of awake prone ventilation therapy. Conclusion Healthcare professionals should pay attention to experience and needs of patients with awake prone positioning ventilation,adopt targeted interventions,and improve the treatment process of awake prone positioning ventilation,with to implement higher quality prone positioning ventilation for patients.

    The article summarized the nursing experience of prone position ventilation under extracorporeal membrane oxygenation(ECMO) in a premature infant with acute respiratory distress syndrome. Nursing key points included establishing the multidisciplinary team to assess the timing of ECMO,development and implementation of individualized prone position ventilation,standardized management of various pipelines to prevent slippage,close observation of vital signs to ensure hemodynamic stability,bleeding and anticoagulation management,refined skin management,prevention of pressure injury and etc. After careful treatment and care,the patient successfully withdrew from ECMO,improved respiratory morphology,and the effect of prone ventilation was significant. After 78 days of hospitalization,the patient was discharged smoothly and in good condition during follow-up.

    Objective To formulate the expert consensus on nursing management of trauma resuscitation units in emergency rooms,providing a reference for standardizing trauma center nursing management and improving the quality of care for severe trauma patients. Methods Based on previous research and an analysis of the needs for trauma unit development,this consensus was developed by reviewing domestic and international studies,guidelines,expert consensus,and clinical practices related to trauma resuscitation unit construction and nursing management over the past ten years. An evidence-based analysis was conducted,and the final consensus was established through a Delphi expert consultation process. Results The consensus consists of 31 recommendations in five key areas,including definition of concepts,configuration and functions of the trauma resuscitation unit,construction and management of basic functions in trauma resuscitation units,construction and management of trauma nursing teams,and development and management of trauma nursing systems. Among these,there were 25 recommendations with Grade A,and 6 with Grade B. Conclusion This consensus is scientific and practical,and it provides guidance for the nursing management of trauma resuscitation units in trauma centers.

    Objective To develop an expert consensus on the prevention and nursing care of postoperative delirium in adults(hereinafter referred to as the“consensus”),to guide the nursing of postoperative delirium in adults. Methods The guidelines,expert consensuses,systematic reviews,and original studies related to postoperative delirium in adults were searched in domestic and international databases and related websites,with the search period from the establishment of the database to May 2024. The first draft of the consensus was written through literature review,stakeholder interviews and expert meeting. From December 2024 to January 2025,19 experts were invited to conduct two rounds of letter consultation,revise and improve the content of the first draft,determine the level of evidence and recommendation strength,and form the final draft of the consensus. Results The content of the “consensus” includes five aspects:classification,risk factors,assessment and identification,prevention,and nursing care of postoperative delirium in adults. Conclusion This consensus can provide practical guidance for clinical nursing staff in the prevention and nursing care of postoperative delirium in adults.

    To summarize the nursing experience of a case of acute myocardial infarction with euglycemic diabetic ketoacidosis complicated with cerebral hemorrhage. According to the characteristics of atypical symptoms,high risk of the disease and rarity of euglycemic diabetic ketoacidosis,measures such as observation and identification of atypical symptoms,systematic evaluation of the risk of ketosis,goal-oriented control of blood glucose,refined fluid management,correction of internal environment disorders,assessment of thrombi and bleeding risk,psychological nursing with emotional ABC management theory were adopted. After 16 days of comprehensive treatment and careful nursing,the patient’s condition was stable with good mood and discharged.

    To summarize the nursing experience of a patient with severe wasp sting-induced multiple organ dysfunction syndrome. A patient suffered from severe wasp stings in a short period of time,which led to rapid liver,kidney,and hematological system damage,and rapid disease progression,needed to maintain stable internal environment,and prevent organ function deterioration. The following measures were taken,including the early removal of toxins,development of an optimal blood purification and anticoagulation regimen,dynamic monitoring of multi-organ function indicators and implementation of effective measures to protect target organs,optimization of the nutrition plan to promote tissue and organ function recovery,and application of traditional Chinese medicine techniques to improve sleep and promote tissue repair. After 12 days of meticulous treatment and nursing care,the patient improved and was discharged from the hospital.

    To summarize the nursing experience of a patient with citrullinemia type Ⅰ complicated by chylorrhea after liver transplantation. In view of the psychiatric disorders caused by acute episodes of “hyperammonemia" in the child,chylorrhea after liver transplantation,difficult control of tacrolimus blood concentration,feeding difficulties,and etc.,measures such as setting up multidisciplinary collaborative team,predictive management of blood ammonia,phased nursing intervention management of chylorrhea,refined tacrolimus nursing,feeding nursing based on sucking function were adopted. After 51 days of treatment and nursing,the patient was discharged from hospital in a stable condition. The patient was followed up for 6 months after discharge and was in good condition.

    To summarize the nursing experience of chimeric antigen receptor T-cell(CAR-T) immunotherapy in a child with systemic lupus erythematosus. The key nursing points included:setting up a multidisciplinary team before infusion to develop a personalized assessment scale and implement high risk prevention,precise reinfusion of CAR T-cells and close monitoring,strengthening the management of adverse reactions such as cytokine release syndrome and immune effector cell-related neurotoxic syndrome after infusion. At the same time,the team paied attention to children’s psychology and did fun narrative nursing. Through individualized treatment and nursing,the child’s condition was effectively controlled and discharged smoothly. Three months after transfusion,the disease activity index of systemic lupus erythematosus decreased to 0,and the 24-hour urine protein decreased to 62.5 mg.

    To summarize the postoperative nursing experience of two patients with anti-melanoma differentiation-associated gene 5 antibody-positive dermatomyositis(MDA5-DM) who underwent lung transplantation. To address the problems of infection exacerbation,primary graft dysfunction and cardiovascular complications which were likely to occur after lung transplantation in these patients with anti-MDA5-DM,a number of targeted nursing measures were taken,including multifaceted infection prevention and control,prevention and management of complications,early rehabilitation and exercise,and monitoring of the primary disease and the side effects of immunosppressive drugs. After careful treatment and nursing care,both patients were transferred to the general ward after their conditions were stabilized. A follow-up conducted 2 months after ICU discharge showed patients were in good condition.

    Best evidence practice of quality improvement in volume management in myocardial infarction patients during the acute phase

    Objective To apply the best evidence on volume management of myocardial infarction patients during the acute phase to clinical practice and evaluate its effect. Methods The team summarized the best evidence for volume management in patients with acute myocardial infarction,developed evaluation indicators based on the best evidence,analyzed the influencing factors of evidence translation,and developed implementation plans in combination with clinical scenarios. Evidence based clinical practice was implemented from September 2023 to December 2023 in cardiovascular wards of two campuses of a tertiary class A hospital in Chongqing. The knowledge level of medical staff on volume management of acute myocardial infarction patients and the implementation rate of review indicators were compared before and after the practice,as well as the improvement of patient related outcome indicators. Results Before and after the application of evidence,the nurses’ volume management knowledge score improved from (80.13±11.97) to (89.37±14.91)(P<0.001),and the implementation rate of the 11 review indicators improved from 15.38%-57.69% to 66.67%-100%(P<0.05). Patients’ NYHA cardiac function grade,number of pulmonary B-lines,left ventricular ejection fraction,NT-proBNP,maximum diameter of inferior vena cava,length of hospital stay,and unplanned readmission rate within 3 months improved(P<0.05),while six-minute walking distance did not improve(P>0.05). Conclusion Applying an evidence-based program for volume management in the acute phase of myocardial infarction patients improves patient prognosis and increases nurses’ adherence to best evidence implementation.

    Risk prediction models for subsyndromal delirium in ICU patients:a systematic review

    Objective To systematically retrieve and evaluate the subsyndromal delirium (SSD) risk prediction models for ICU patients,aiming to provide reference for medical staff to construct and select scientifically effective risk prediction models. Methods The researches on SSD risk prediction models for ICU patients were retrieved in PubMed,Embase,Web of Science,Cochrane Library,CINAHL,China National Knowledge Infrastructure,VIP database,Wanfang database,and China Biomedical Literature Database,the retrieval time limit was from the establishement of database until July 19,2024. Two researchers independently screened literature,extracted data,and evaluated the bias risk and applicability of the included literature. Results A total of 8 studies on SSD risk prediction model for ICU patients were included,involving 8 models,with an area under the curve(AUC) of 0.710-0.956 for establishing the models. The overall applicability was good,but the risk of bias was high,mainly concentrated in the field of data analysis. Conclusion The existing ICU patient SSD risk prediction models have good overall performance,but have a high risk of bias. In the future,the existing models can be further optimized or higher quality prediction models can be developed.

    Objective To conduct a comprehensive search for evidence relating to skin care for adults with Stevens-Johnson syndrome and toxic epidermal necrolysis and summarize the best evidence. Methods Systematic search of domestic and international databases,guideline websites,and websites of dermatology-related societies was conducted for clinical decisions,guidelines,evidence summaries,expert consensus,systematic reviews,and original research on skin care for adults with Stevens-Johnson syndrome and toxic epidermal necrolysis. The search timeframe was from January 2014 to September 2024. Quality assessment of the literature and extraction of information was performed independently by 4 investigators. Results A total of 12 papers were included,including one clinical decision,five guidelines,two systematic reviews,three expert consensuses,and one original study. Thirty-nine pieces of evidence encompassing five topics were summarized,including formation of multidisciplinary teams,assessment and monitoring,skin barrier care,mucosal system care,health education and follow-up. Conclusion This study summarizes the best evidence for skin care in adults with Stevens-Johnson syndrome and toxic epidermal necrolysis,can provide an evidence-based basis for clinical implementation of skin care.

    A visualized analysis of research hotspots and development trend on fall prevention and control in older adults

    Objective To analyze the research hotspots and development trends of fall prevention and control among the elderly,providing references for healthcare professionals to implement effective prevention and intervention strategies. Methods A systematic search was conducted for relevant literature on fall prevention and control among the elderly in CNKI database,covering the time frame from June 1,2014,to June 1,2024. Bibliometric analysis was performed using CiteSpace 6.3.1 software. Results A total of 1,406 articles were included,showing an overall upward trend in publication volume. Keyword co-occurrence analysis revealed that research hotspots were concentrated in areas such as fall risk factors,fall detection technologies,fear of falling,and fall interventions. The research has formed eight clusters,including validity,elderly,fall detection,risk factors,fear of falling,fall risk,balance ability,and injuries. Keyword burst analysis indicated that future research would focus on technology-driven personalized prevention,interdisciplinary collaboration,and community involvement,promoting the comprehensive development of precise intervention strategies. Conclusion Research on fall prevention and control among the elderly has been expanding. In the future,it is necessary to further strengthen collaboration among research institutions,foster interdisciplinary communication,and advance personalized fall prevention strategies alongside intelligent technologies,in order to enhance the scientific and effective prevention and control of falls in the elderly.

    The application of decision aids for patients with life-support treatment in ICU:a scoping review

    Objective To conduct a scoping review of decision aids(DA) for patients with life-support treatment in ICU,identify the application status of DA in ICU,content elements and application effects,provide guidance for the development of DA for life support therapy in ICU patients in China. Methods PubMed,Web of Science,Embase,Cochrane Library,CNKI,Wanfang database,CBM and VIP database were searched. The retrieval time limit was from the establishment of databases until May 7,2024. The included literature was summarized and analyzed. Results A total of 21 articles were included,and topics on DA included specific diseases,life-sustaining treatments,advance care planning,hospice care. The content elements of DA included clear treatment options,information support,risks and benefits associated with treatment options,clarifying values and guiding decision-making. The evaluation indexes included the quality of the decision making process,decision quality,the impact of DA on patients and medical institutions,feasibility,usability and acceptability. Conclusion DA has shown good effects in ICU life support treatment. DA mainly focuses on specific diseases,life support treatment and nursing goals in ICU,while most patients coexist with multiple diseases,the life support treatment measures are not limited to one. We should fully consider the domestic situation and develop DA suitable for ICU patients with life-support treatment in China to meet the individual needs of different groups.

    Research progress of end tidal carbon dioxide monitoring in patients undergoing cardiopulmonary resuscitation

    Cardiac arrest is one of the most common and critical diseases in the emergency department,and it is also an important health problem of public concern. If not treated in time,it will cause serious damage to the tissues and functional organs of human,and even threaten the lives of patients. Cardiopulmonary resuscitation,as an important treatment method for cardiac arrest,is the key to save patients’ lives,but the success rate of resuscitation is generally low at this stage. In order to improve the success rate of treatment of patients with cardiac arrest,high-quality cardiopulmonary resuscitation and effective index monitoring in the process of cardiopulmonary resuscitation to guide the quality of resuscitation were proposed. This article reviews the monitoring principle and waveform interpretation of the partial pressure of end tidal carbon dioxide (PETCO2),the application guidance value in cardiopulmonary resuscitation,and the factors affecting PETCO2 during cardiopulmonary resuscitation,in order to provide reference for the application of PETCO2 monitoring in cardiopulmonary resuscitation.
