    The construction of out-of-hospital cardiac arrest database

    Objective To construet the out-of hospital cardiac arrest (OHCA) database based on Utstein style for standardizing the ollection of data and evaluation of the efct of cardiopulmonary resuscitation,so as to ffctively improve the quality of cardiopulmonary resuscitation and the survival rate of OHCA patients. Methods Based on, AHA guidelines , European Resuscitation Council guidelines and OHCA databases , we collected OHCA emergency medical records from January 2012 to July 2017 in Pudong New Area and the OHCA database based on Utstein style was constructed with the use of MySQL software and under the guidance of E-R model as well as the third paradigm. The database table structure was determined by expert group meetings. Results The OHCA database was constructed based on Utstein style , which consisted of five modules , namely basic data table , cardiac arrest data table , resuscitation table , outcome table and important time table. The database had the functions of input , export , inquiry and management and included 16 122 medical records. It standardized the input and expression of OHCA data. It has clear structure detailed content and convenient operation , which can provide a template for the routine OHCA registration in Pre-hospital Emergency Center. Conclusion The OHCA database based on Utstein style establishes the OHCA data collection standard in line with the characteristics of our country and provides an accurate management platform of OHCA information. It is not only laying the foundation for the future accurate research of OHCA,but also provides a scientific basis for the formulation of policies related to OHCA treatment.

    Application of standardized admission procedure in shortening aviation

    medical rescue response time

    Objective Apply the standardized aviation medical rescue admission procedure to shorten the aviationmedical rescue response time and improve rescue efficiency. Methods we selected 81 cases of aviation medical rescue cases that were sent to our hospital through 96195 service platform/public platform from January 2017 to November 2019. Among them, 40 patients who received the standardized procedure from July 1,2018 to November 30, 2019 were considered as the experimental group, 4cases from January 1,2017 to May 31,2018 were the control group and received routine procedures. Compared the response time of aviation medical rescue, the success rate of rescue,the correet rate of the disease assessment by doctors before the transfer, and the integrity rate of preparing rescue supplies by nurses. Results After the implementation of the standardized process , the time of illness assessment by doctors , the preparation time of rescue supplies ,the time of medical staff arrival at the apron, and the average response time of the rescue in experimental group were significantly shorter than those of the control group, and the differences were statistically signifcant (P<0.05). The success rate of aviation medical transport, the correct rate of the assessment by doctors before the transfer,and the integrity rate of preparing the rescue supplies by nurses in the experimental group were significantly higher than the control group, and the differences were sta-

    tistically significant (P<0.05). Conclusion The implementation of the standardized aviation medical rescue admission procedure can effectively shorten the rescue response time , thereby improve the rescue efficiency and the success rate of rescue.

    Research on the frequency of oral care in critical children with

    mechanical ventilation

    Objective To analyze the oral status of children with tracheal intubation in different time periods afteroral care with chlorhexidine,and to select the best oral care plan. Methods 63 children who met the included criteria in the Children Intensive Care Unit were selected. The patients received oral care with chlorhexidine by solution washing and scrubbing, pharyngeal swab culture was carried out for the oral cavity before oral care ,2 hours,4 hours,6 hours and 8 hours after oral care respectively. Results According to the analysis of swab results,15 cases had pathogenic bacteria in the oral cavity. The number of bacterial strains was lowest 2 hours after oral care ,and highest 8 hours after oral care. The number of bacterial strains increased with time ( x2=1.825,P=0.171). The rise was slow 2~6 hours after oral care ,and the rise trend became rapid 6~8 hours after oral care.The total number of oral bacteria decreased dramatically 2 hours after oral care. After oral care , the total number of bacterial strains increased with time , showing a stable increasing trend (x=4.841 ,P=0.028). Conclusion Oral care by chlorhexidine solution washing and scrubbing every 6 hours will improve the oral bacteria status of children with mechanical ventilation.

    Delirium is common in severe patients , elderly patients , postoperative patients,adult cancer patients. The incidence is high and it increases mortality. Based on the evidence of the latest guidelines, this paper focused on the management of adult delirium nursing, the definition and lasification, recognition and asessment of delirium, non-pharmacological intervention and other aspects, in order to provide guidance for clinical nurses to reduce the risk of delrium.

    ICU delirium

    Objective To explore the ffects of mutiple blocking model for safety gap in ICU delirium prevention. Methods Patients in the comprehensive ICU of a tertiary first-class hospital in Qingdao from January to October of 2019 were included in the study , and the quasi experimental design was adopted. Through retrospective analysis and interview, the gap factors contributing to ICU delirium were found. And gap blocking measures were formulated based on evidence-based nursing practice. The incidence of delirium before and after gap blocking, the psychological distress degree of family members and doctors and nurses' score of delirium prevention knowledge and humanistic care awareness were compared. Results The delirium incidence after gap blocking was 47.63%  decreased by 15.31%. Delirium duration was(4.32 +0.67 )days , decreased by(0.85+0.08) days. The psychological distress score of the family members , delirium prevention knowledge and humanistic care awareness score of doctors and nurses were statistically significant(P<0.05). Conclusion The application of multiple blocking model for safety gap can reduce the incidence of delirium in ICU ,reduce the psychological distress degree of family members and improve the knowledge level and humanistie care awareness of medical staff.

    目的:梳理国 内外ICU后综合征研究现状及热点趋势,以期对我国ICU后综合征研究的发展起到一定的指导作用。方法WebofScience文献数据平台收录的文献数据为基础,利用文献计量学方法和CiteSpace信息可视化软件,对截至2019101日收录的ICU后综合征相关文献进行计量学分析。结果 检索获得ICU后综合征相关文献1055,发文量呈逐年增加趋势。其中美国438,占总文献量的41.5%,其次是英国113(10.7%)、法国110(10.4%)、中国27(2.6%)。文献发表于53本期刊.,Intensive Care Medicine发 文量最多(60) ,Crtical CareMedicine发文次之(47),Critical Care(42)第三,以危重病医学类专业型期刊居多;本研究共得到14个名词术语聚类及共词的名词短语,经过分析发现目前ICU后综合征的研究尚停留在流行病学和影响因素调查的层面。结论近年来,ICU后综合征逐渐引起国内外学者的重视。ICU后综合征的研究方向较局限且发表集中在欧美发达国家,ICU后综合征流行病学及影响因素为研究热点。目前国内关于ICU后综合征研究鲜有报道,今后应加强对ICU转出患者管理的延续性研究。
