摘要:

呼吸机相关性肺炎(ventilator-associated pneumonia,VAP)作为ICU经口气管插管机械通气患者最为严重的并发症之一,会显著延长患者住院时长、增加医疗成本并提高病死率。规范化的口腔护理能够有效降低VAP的发生率,然而当前临床实践仍面临三大核心矛盾:传统评估工具在ICU患者中的适用性欠佳;氯己定溶液浓度的选择在效能与安全性方面存在争议;口腔护理的临床执行率普遍较低。为解决上述问题,该文系统整合近10年相关文献,梳理机械通气患者口腔护理在“评估-工具-溶液-频率”4个维度的研究进展,归纳提出“评估-匹配-调整”三步递进式临床决策路径。该路径融合“人工智能视觉识别系统+口腔护理频率评估量表”以实现精准评估,依据风险分层与资源条件匹配选择护理工具及口腔护理溶液,并通过口腔护理频率评估量表评分动态调整护理频率,旨在为临床提供结构化、个体化的实践策略,推动口腔护理向标准化与精准化转变。然而,该决策路径的综合效能及其在不同医疗资源环境中的适配性,仍需通过大样本、多中心的临床研究进一步验证与优化。

Abstract:

Ventilator-associated pneumonia(VAP) is one of the most severe complications in ICU patients undergoing orotracheal intubation mechanical ventilation,significantly prolonging hospital stay,increasing medical costs,and elevating mortality rates. Standardized oral care can effectively reduce the incidence of VAP,current clinical practice still faces three core challenges:the inadequate applicability of traditional assessment tools for ICU patients,the controversy regarding the efficacy and safety of chlorhexidine solution concentration selection,and the generally low clinical implementation rate of oral care. To address these issues,this article systematically integrates evidence from the past decade literature,reviews the research progress in four dimensions of oral care for mechanical ventilation patients—“assessment,tool,solution,frequency”,and summarizes a three-step progressive clinical decision-making pathway termed “assessment-matching-adjustment.” This pathway integrates artificial intelligence visual recognition system and Intensive Care Oral Care Frequency Assessment Scale(ICOCFAS) to achieve precise assessment,matches nursing tools and solutions based on risk stratification and resource conditions,and dynamically adjusts the frequency of care via ICOCFAS scores. It aims to provide a structured,individualized practical strategy for clinical practice,promoting the transition of oral care toward standardization and precision. However,the overall efficacy of this decision-making pathway and its adaptability across different healthcare resource environments still require further validation and optimization through large-sample,multicenter real-world clinical studies.
