ICU成人患者人工气道湿化管理的最佳证据总结

Summary of best evidence for humidification management of artificial airways in adult ICU patients

摘要: 目的 检索、评价并总结国内外ICU成人患者人工气道湿化管理的最佳证据,为临床实践提供参考。 方法 计算机检索国内外相关数据库及网站关于ICU成人患者人工气道湿化管理的相关文献,筛选出符合纳入标准的文献并对其进行质量评价及证据提取。检索时限为建库至2022年4月。结果 共纳入13篇文献,其中临床决策1篇、指南3篇、系统评价5篇、证据总结2篇、专家共识2篇。分别从人工气道湿化的适用范围、湿化装置、湿化方式、湿化液、评估与监测、辅助疗法、人员管理7个维度,形成15条最佳证据。结论 本研究总结了ICU成人患者人工气道湿化管理的最佳证据,医护人员在应用前还需结合临床实际情况,有针对性地选择证据并规范应用,从而提高气道管理效果,降低患者人工气道相关并发症的发生率。

Abstract: Objective To search,evaluate,and summarize the best evidence on artificial airway humidification management for adult ICU patients at home and abroad,and to provide a reference for clinical practice. Methods We searched all relevant databases and websites about all relevant literature related to the management of airway humidification in adult ICU patients,then selected the literature that met the inclusion criteria,evaluated their quality and extracted evidence. The retrieval period was from the inception of databases to April 2022. Results A total of 13 articles were included, including 1 clinical decision, 3 guidelines, 5 systematic reviews, 2 evidence summaries, and 2 expert consensuses. From 7 dimensions including the scope of application of artificial airway humidification,humidification device, humidification method,humidification fluid, assessment and monitoring,auxiliary therapy,and personnel organization,15 best evidences were summarized. Conclusion This study summarizes the best evidence for the management of airway humidification in adult ICU patients. Medical staff should also select the evidence in a targeted manner based on the actual clinical situation before application,to improve the effects of airway humidification and reduce the incidence of airway complications in patients.
