急危重症护士道德困境的范围综述

A scoping review of moral distress of emergency and critical care nurses

摘要: 目的 了解国内外急危重症护士道德困境的研究现况。方法 检索PubMed、Embase、The Cochrane Library、CINAHL、Scopus、中国生物医学文献数据库、中国知网、维普中文科技期刊数据库、万方数据库中相关文献,检索时限为建库至2021年3月,并进行文献筛选和数据提取。结果 共纳入35篇文献,结果显示,急危重症护士道德困境的影响因素包括一般人口学特征、个人内部因素、外部因素、特定临床情境等。研究中应对护士道德困境的干预策略主要包括小组讨论、沙龙活动、课程讲座、叙事性写作等。干预后护士道德困境改变的统计学差异尚未得出一致结果。结论 多种因素对急危重症护士的道德困境产生影响,需采取多种形式相结合且适合国情与文化的干预策略。

Abstract: Objective To understand the research status of moral distress of emergency and critical care nurses at home and abroad. Methods PubMed,EMBASE,the Cochrane Library,CINAHL,Scopus,China biomedical literature database,CNKI,VIP Chinese journal database and Wanfang database were searched from the establishment of the database to March 2021. Literature screening and data extraction were carried out. Results A total of 35 literatures were included. The results showed that the influencing factors of moral distress of emergency and critical care nurses included general demographic characteristics,personal internal factors,external factors and specific clinical situations. Different intervention strategies were adopted to deal with the moral distress of nurses,including group discussion,salon activities,course lectures,narrative writing and so on. Before and after the intervention,the changes of nurses' moral distress have not been consistent. Conclusion Many factors have an impact on the moral distress of nurses in emergency and critical care. Intervention strategies that combine various forms and are suitable for national conditions and culture should be adopted.
