虚拟现实技术在成人ICU患者中应用的范围综述

Application of the virtual reality technology in adult ICU patients:a scoping review

摘要: 目的 本研究对虚拟现实(virtual reality,VR)技术在成人ICU患者中应用的相关研究进行范围综述,识别干预的具体内容、结果及局限性,为医护人员开展相关研究或在临床应用该技术提供参考。方法 根据范围综述的研究方法,计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、Cochrane Library、PubMed、Embase、Web of Science、CINAHL、Scopus、Proquest、IEEE Xplore等国内外数据库,检索时限为建库至2023年3月11日。由2名研究者独立筛选文献、提取数据。结果 共纳入23篇文献,9篇随机对照试验,12篇类实验研究,2篇观察性研究。VR技术为成人ICU患者提供放松和康复干预,对患者的运动功能康复有积极影响,对预防谵妄有潜在效果,但受样本量、周期、环境等局限,能否改善认知、心理、睡眠、疼痛尚存争议。未有文献报告严重不良事件,患者接受度较好。结论 VR技术在成人ICU患者中应用可行且效果积极。未来应结合“互联网+”护理服务优势,以患者为中心,将技术创新与人文护理虚实结合,规范评价标准,探究更全面、个性化的干预策略。

Abstract: Objective To conduct a scoping review to identify the specific intervention,results and limitations of virtual reality(VR)technology in adult ICU patients,so as to provide the guidance for future research and clinical application. Methods A literature search was performed in the CNKI,Wanfang database,VIP,CBM,Cochrane Library,PubMed,Embase,Web of Science,CINAHL,Scopus,Proquest and IEEE Xplore to identify relevant studies. The retrial limit was from database establishment to March 11,2023. The studies were screened and data was extrated independently by two researchers. Results Totally 23 studies were included,including nine randomized controlled trials,two quasi-experimental studies and two observational studies. Virtual reality(VR)technology provided relaxation and rehabilitation intervention for adult ICU patients. The results showed that VR interventions had positive effects on the motor function rehabilitation of patients,and had a potential effect on the prevention of delirium. However,due to the limitations of sample size,intervention periodicity,environment,whether it can improve cognition,psychology,sleep and pain was still controversial. No serious adverse events were reported and the acceptability of VR technology intervention in patients was good. Conclusion Virtual reality technology is feasible and has a positive effect on adult ICU patients. Future research should combine the advantages of “Internet plus nursing service”,take patients as the center,combine technological innovation and humanistic nursing,to formulate standardized evaluation standards and explore more comprehensive and personalized intervention strategies.
