    Evidence transformation and effect evaluation of glycemic monitoring and multidimensional management in adult ICU patients

    Objective Guided by the Evidence Implementation Model developed by the Fudan University Centre for Evidence-Based Nursing,this study aimed to apply the best evidence for glycemic management in critically ill patients and evaluate its effectiveness. Methods A four-phase approach—comprising evidence retrieval,baseline audit,practice innovation,and outcome evaluation—was employed to implement best practice in glycemic management in the ICU of a tertiary class A hospital in Zhejiang Province. Outcomes were compared before(June-July 2024) and after(October-November 2024) the evidence application,focusing on healthcare providers’ compliance with audit criteria,the proportion of patients within the time in target blood glucose range,glucose variability,and the incidence of hypoglycemia. Results The study included 8 physicians,28 nurses,and 60 patients(30 before and 30 after the intervention). After the evidence practice,there was a significant improvement in the execution rates of audit criteria among clinicians,with 12 audit indicators showing statistically significant differences compared to baseline(P<0.05). System-level audit criteria(items 10,24) achieved 100% compliance. The differences in the average proportion of patients within the time in target blood glucose range and glucose variability before and after the evidence-based practice were statistically significant(P=0.026,0.027). However,the difference in the incidence of hypoglycemia was not statistically significant(P=0.667). Conclusion The evidence-based practice for blood glucose management effectively standardized the behaviors of healthcare providers,improved the proportion of critically ill patients within the target blood glucose range,reduced glucose variability and the incidence of hypoglycemia.

    Transformation of the best evidence and effect evaluation of stepwise physical restraint in neurocritical care patients

    Objective To apply the best evidence of stepwise physical restraint protocol for critically ill neurological patients in clinical practice,guided by a humanistic care model,and to evaluate its effectiveness. Methods The best evidence regarding stepwise physical restraint for critically ill neurological patients was systematically reviewed. Evidence-based nursing protocols were developed,baseline assessments were conducted to identify potential barriers,and a best implementation strategy was formulated. A stepwise physical restraint program incorporating the principles of humanistic care was designed. This program was implemented in the neurosurgical intensive care unit(ICU) of a Class Ⅲ Grade A hospital in Jiangsu Province. The compliance rates of various audit indicators,the duration of physical restraint,and the incidence of related complications were compared before and after the implementation of the evidence. Results There were statistically significant improvements in the implementation of 26 indicators following the application of the evidence. Statistically significant differences were observed in the physical restraint rate,total restraint time,first and second restraint durations,and negative patient experiences associated with restraint between the two groups(P<0.05). The reduction in physical restraint did not lead to an increase in unplanned extubation. Conclusion The application of best evidence of stepwise physical restraint for critically ill neurological patients can effectively shorten restraint duration,reduce restraint rates,and ensure patient safety.

    Analysis of influencing factors on the recovery of spontaneous circulation in patients with cardiogenic in-hospital cardiac arrest

    Objective To explore the relevant influencing factors of spontaneous circulation recovery in patients with cardiogenicin-hospital cardiac arrest,and provide reference for preventing cardiac arrest and improving the success rate of rescue. Methods A retrospective analysis was conducted to continuously collect clinical data of 95 patients with cardiogenic in-hospital cardiac arrest admitted to a tertiary class A hospital in Zhenjiang City from May 2019 to May 2024. Based on the rescue outcomes,the patients were divided into a group with return of spontaneous circulation(ROSC) and a group without ROSC. Multivariate logistic regression analysis was conducted to explore the influencing factors. Results Among the 95 patients,61 recovered spontaneous circulation and 34 did not,with a success rate of 64.2%. Logistic regression analysis showed that premature contractions(OR=4.924,95% CI:1.368-17.723,P=0.015),nighttime(OR=4.805,95%CI:1.258-18.357,P=0.022),and cardiopulmonary resuscitation duration>15 minutes(OR=13.049,95%CI:1.424-119.602,P=0.023) were risk factors for spontaneous circulation recovery in patients with in-hospital cardiac arrest. Initial chest tightness and chest pain symptoms(OR=0.222,95% CI:0.057-0.864,P=0.030),tracheal intubation(OR=0.051,95%CI:0.009-0.282,P=0.001) were protective factors for the recovery of spontaneous circulation in patients with in-hospital cardiac arrest. Conclusion The success rate of ROSC in patients with cardiogenic in-hospital cardiac arrest is relatively low. Nursing staff should pay attention to patients with initial symptoms such as chest tightness and chest pain,and premature contractions before cardiac arrest. The professional strength of night shift nurses should be strengthened to increase the frequency of patrols,timely detect symptoms and arrhythmias of cardiac arrest,and activate rapid emergency teams to improve the success rate of ROSC for cardiogenic in-hospital cardiac arrest patients.

    Construction and application of a prevention and graded management plan for skin tears in post-cardiac surgery patients

    Objective To establish and implement a skin tear prevention and graded management plan for post-cardiac surgery patients. Methods Based on risk management theory and hierarchical nursing theory,an initial skin tear prevention and graded management plan for post-cardiac surgery patients was drafted through literature review and panel discussions,followed by finalization via an expert consensus meeting. Using a non-randomized controlled trial design,266 patients(September-November 2024) were allocated to the experimental group,while 254 patients(June-August 2024) were enrolled in the control group. The experimental group received a prevention and grading management plan,while the control group received routine skin care. A comparison was made between the two groups regarding skin tear incidence,severity,skincare costs,healing time,and nurses’ theoretical and practical assessment scores on skin tear management. Results The experimental group had significantly lower skin tear incidence,severity,healing time,and costs(P<0.05),while nurses achieved higher rates of timely/accurate risk assessment,intervention adherence,and test scores(P<0.05). Conclusion The prevention and graded management plan for skin tears in postoperative cardiac patients is scientific and feasible. It can reduce the incidence and severity of skin tears,decrease nursing workload,lower skin care costs,and enhance nurses’ awareness and management capabilities regarding skin tears,achieving predictive risk-based nursing for skin tears.

    Current status and influencing factors of self-management ability of pulmonary rehabilitation training in patients after mechanical heart valve replacement surgery

    Objective To analyze the current status and influencing factors of self-management ability of pulmonary rehabilitation training in patients after mechanical heart valve replacement surgery,and provide reference for the development of effective pulmonary rehabilitation intervention plans. Methods Through convenience sampling,201 eligible patients who underwent mechanical heart valve replacement surgery at a tertiary hospital in Yunnan Province from November 2023 to March 2024 were selected as the study subjects. A comprehensive survey was conducted on patients using a general information questionnaire,a self-management scale for patients after mechanical heart valve replacement surgery,a pulmonary rehabilitation self-efficacy index scale,and a concise quality of life scale. Through multiple stepwise regression analysis,factors affecting the self-management ability of lung rehabilitation training in patients after mechanical heart valve replacement surgery were identified. Results Multiple stepwise regression analysis showed that gender,age,average monthly household income,educational level,presence of other chronic diseases,self-efficacy in pulmonary rehabilitation,and quality of life were the main influencing factors on the self-management ability of pulmonary rehabilitation training in patients after cardiac mechanical valve replacement surgery. Conclusion The self-management ability of pulmonary rehabilitation training for patients undergoing mechanical heart valve replacement surgery urgently needs to be improved,and is influenced by many factors. Medical staff should take various measures to improve the self-management ability of patients undergoing mechanical heart valve replacement surgery,thereby enhancing self-efficacy and improving quality of life,providing scientific basis for reducing the occurrence of postoperative pulmonary complications.

    To summarize the nursing experience of a child who developed ventricular fibrillation storm after cardiac valve replacement surgery and was successfully treated by extracorporeal cardiopulmonary resuscitation. In critical situations where defibrillation was repeated and drug therapy was ineffective,the emergency team for extracorporeal membrane oxygenation after cardiac surgery was quickly activated to implement cardiopulmonary resuscitation. Subsequently,targeted temperature management,neuroprotective strategies,and goal-directed analgesia and sedation protocols were implemented. Through multidisciplinary team collaboration and meticulous nursing care,the child was successfully liberated from ECMO on postoperative day(POD) 7. He was transferred from ICU to the cardiac surgery on POD 12,and was discharged on POD 19. The follow-up showed good prognosis.

    Analysis of influencing factors of driveline infection after left ventricular assist implantation and nursing enlightenment

    Objective To explore the influencing factors of driveline infection(DLI) after left ventricular assist device(LVAD) implantation,and put forward targeted nursing countermeasures. Methods A total of 120 patients with LVAD implantation from four medical centers in China from July 2021 to December 2024 were retrospectively selected. The data were collected by the general information questionnaire,the Appraisal of Self-care Agency Scale-Revised(ASAS-R-C) and the Family APGAR Index Questionnaire. Binary logistic regression was used to analyze the determinants of DLI. Results The incidence of DLI was 19.17%(23/120). Univariate analysis showed that diabetes(OR=7.320,P<0.001),low self-care ability(OR=0.893,P<0.001) and low family care index(OR=0.450,P<0.001) were significantly correlated with DLI. Multivariate analysis showed that diabetes was an independent risk factor for DLI(OR=6.050,P=0.003),and family care index was a protective factor(OR=0.533,P=0.014). Conclusion The occurrence of DLI is closely related to diabetes,self-care ability and family support. It is recommended to reduce the incidence of DLI by blood glucose control,improving self-management ability and family care.

    Nursing of the first case of axial flow left ventricular assist device bridging to heart transplantation in China

    This report summarized the nursing experience of a pediatric patient who underwent implantation of an axial-flow left ventricular assist device(LVAD) as a bridge to heart transplantation. As the first domestic application of an axial-flow LVAD specifically designed for low-weight children,due to the absence of prior therapeutic and nursing references. A multidisciplinary care team was established to provide comprehensive postoperative evaluation and management. Pump speed was precisely set and dynamically adjusted according to the patient’s body surface area and target cardiac output. Trends in cardiac function recovery were assessed by monitoring pulse pressure changes alongside circulatory parameters and patient’s condition. Notably,when pump speed remained constant,a transient increase in power exceeding 2W suggested the possibility of thrombus formation or embolization in the circuit. Given the characteristics of axial flow pumps causing significant red blood cell damage and reactive activation of the body’s anticoagulation system,a goal-directed anticoagulation strategy was adopted. Based on the family’s specific circumstances,media reports were utilized to mobilize social support for the child’s treatment,successfully bridging the case to a heart transplant.

    Dyadic disease management refers to treating the patient and their caregiver as an interdependent unit,jointly managing the disease. Left ventricular assist device implantation has become an alternative treatment for heart failure patients awaiting heart transplantation or for myocardial functional recovery. However,the complex daily disease management and postoperative complications impose significant burdens on both patients and their caregivers. This article reviews the current status of factors influencing dyadic involvement in disease decision-making,managing daily health behaviors,and emotional support for left ventricular assist device recipients and their caregivers,as well as existing dyadic disease management interventions. The aim is to enhance clinical nursing staff’s awareness of dyadic coping,provide insights for addressing comprehensive care needs,and inform the development of dyadic coping intervention strategies.

    Perioperative nursing of the first spinal cord injury patient treated with a closed-loop spinal cord neural interface system in China

    To summarize the perioperative nursing experience of the first spinal cord injury patient treated with a closed-loop spinal cord neural interface system in China. The key nursing points included establishing a comprehensive management team to provide full-spectrum professional support,implementing a phased rehabilitation strategy focused on trunk motor control under the synergistic effect of epidural electrical stimulation(EES),emphasizing neurogenic bladder and bowel management with a scientifically formulated plan to guide stimulator maintenance and management,and providing continuity of care. Through meticulous treatment and nursing,the patient achieved standing with assisted knee locking by postoperative day 15 and was subsequently transferred to a local hospital for further rehabilitation. At the 2-month follow-up,the patient was able to walk 100 meters with a walking aid and step up onto a 5 cm(2 inch) step.

    This article summarized the nursing experience of a patient with severe meningoencephalitis caused by porcine herpesvirus infection,accompanied by acute retinal necrosis in both eyes. While providing antiviral,anti-infective,high-dose hormone shock therapy,sedation,mechanical ventilation and other treatments,we also observed the condition well. We implemented postoperative care for eye surgery,contact isolation care,pulmonary rehabilitation care,and multiple targeted nursing measures. The patient was clinically cured and discharged on the 22nd day after admission.

    Nursing care of severe skin injury in an extremely preterm infant following intestinal perforation surgery

    To summarize the nursing experience of an extremely preterm infant born at 23+2 weeks with a birth weight of 500 grams who developed severe skin injury following surgery for spontaneous intestinal perforation. In response to challenges including postoperative wound exudation and hemorrhage,extensive skin injury,and lack of nursing experience,a multidisciplinary case management team was established to develop a wound care protocol,implement pre- and post-dressing pain assessments and management,implement strictly infection prevention measures. After 29 days of careful treatment and nursing care,the infant’s wounds achieved primary healing with no secondary infections. The infant was successfully discharged after 198 days of hospitalization.

    Investigation on the setting and nursing status of animal injury clinic and emergency department in 23 provinces of China

    Objective To understand the current status of the setting and nursing work in animal injury clinic and emergency department in China,so as to promote the development of the discipline of nursing. Methods A questionnaire was developed through literature review and expert consultation. The questionnaire included basic information and the setting in animal injury clinic,nursing work content and vaccination. Using convenience sampling method,a survey will be conducted from November to December 2023 on nursing staff engaged in animal injury work nationwide. Results A total of 318 valid questionnaires were collected,covering 123 medical institutions in 69 regions of 23 provinces. The results showed that different medical institutions had different names for "animal injury clinic",and the work contents included management of animal injury clinic,triage,wound management,active and passive immune preparation injection,adverse reaction observation,health education,follow-up,science popularization,teaching and scientific research,etc. The main problems encountered by nurses in the work included insufficient knowledge/ability of wound management,insufficient understanding/cooperation of patients,unclear post management,insufficient staffing,and doctor-nurse collaboration to be strengthened. There were many differences in the vaccination of animal injury clinics,including the selection of skin disinfectant during vaccination,the selection of syringe,the site of vaccination,and the rewarming of the vaccine before vaccination. Conclusion There are differences in the settings of hospital animal-induced injury clinic(emergency) departments,the contents of nursing work,and the details of vaccination operations in 23 provinces of our country. A professional academic platform and team should be established to standardize the name of animal injury clinic and the training contents,frequency and assessment of nursing staff,formulate industry standards to standardize vaccination and management. And clear job responsibilities,work content,work flow and operating standards should be formulated,communication and collaboration between doctors and nurses should be strengthened,and the training of nurses’ scientific research ability should be enhanced to promote the continuous development of animal injury nursing.

    Analysis of facilitators and barriers to evidence implementation of minimal physical restraint in ICU children

    Objective To explore the potential facilitators and barriers to the clinical translation of evidence related to minimizing physical restraint in ICU children for formulating change strategies. Methods Based on the evidence-based method,the best evidence was extracted and 13 review indicators and methods were formulated. ICU nurses and children were reviewed in a tertiary class A Children’s Hospital. The promotion guideline list of the i-PARIHS framework was adopted. According to the review indicators and baseline review results,the promoting and barrier factors of evidence transformation were analyzed,and change strategies were formulated. Results The rate of physical restraint before best evidence application in ICU was 71.19%. Among the 13 review indicators,the implementation rate of 3 review indicators was >60%,the implementation rate of the remaining 10 review indicators was less than 60%,and the implementation rate of 6 review indicators was 0. The total score of ICU nurses’ knowledge and practice questionnaire on physical restraint was(55.77±8.11). 15 facilitators and 15 barriers for minimizing physical constraint evidence transformation in ICU children were analyzed,and 11 change strategies were developed. Conclusion There is still a big gap between the best evidence of minimizing physical restraint among children in ICU and clinical practice. Scientific review indicators should be developed,facilitators and barriers factors should be comprehensively analyzed,and corresponding reform strategies should be implemented to lay the foundation for evidence-based nursing practice.

    An analysis of the status quo and influencing factors of perceived professional benefits among pediatric nurses in 130 hospitals in China

    Objective To investigate the current level of perceived professional benefits among pediatric nurses and identify influencing factors,providing evidence for improving their professional mental health. Methods A cross sectional online survey was conducted from August to December 2022 using convenience sampling,using a general information questionnaire and the Nurse’s Perceived Professional Benefits(NPPB)scale. A total of 529 pediatric nurses from 130 children’s hospitals,maternal and child health hospitals,and general hospitals across 31 cities in 15 provinces were enrolled. Univariate analysis and multiple linear regression were performed to identify influencing factors. Results The mean NPPB score was 140.28±19.21,exceeding the theoretical median of 132,indicating a moderate to high level of professional benefit perception. Multiple regression analysis revealed that professional title (β=0.196,P<0.001),relationship with preceptors(β=-0.224,P<0.001),relationship with colleagues(β=-0.106,P=0.039),reasons for choosing nursing as a career(β=-0.176,P<0.001),and receipt of humanistic care education(β= -0.126,P=0.001) were independent predictors. Conclusion Pediatric nurses report a generally favorable level of perceived professional benefits,yet room for improvement remains. Nursing administrators should enhance psychological support and strengthen humanistic care training to improve professional recognition and ensure workforce stability.

    Application of ceiling lifts in early mobilization for ventilated ICU patients

    Objective To evaluate the effects of ceiling lifts on early mobilization in ventilated ICU patients. Methods A total of 104 ventilated patients admitted to the ICU of a tertiary class A hospital in Hangzhou from March 2023 to November 2023 were enrolled and randomized into the experimental group(n=52) and the control group(n=52). The experimental group received ceiling lift-based early mobilization,while the control group received conventional early mobilization. Outcomes were compared between the two groups,including initiation time to first bedside sitting and standing,average daily mobilization dosage,incidence of ICU-acquired weakness,duration of mechanical ventilation,length of ICU stay,safety,as well as patients’ and nurses’ experiences with early mobilization. Results Compared with the control group,the experimental group showed significantly earlier first bedside sitting and standing,higher average daily mobilization dosage,lower ICU-acquired weakness incidence,and greater satisfaction among patients and nurses(all P<0.05). No significant differences were observed in duration of mechanical ventilation,length of ICU stay,or early mobilization-related adverse events(P>0.05). Conclusion Ceiling lifts facilitate earlier out-of-bed mobilization,increase daily mobilization dosage,reduce ICU-acquired weakness,and improve satisfaction among patients and nurses,offering a novel strategy for high-quality rehabilitation in ventilated ICU patients.

    Visual analysis of research hotspots and development trends in palliative care for pediatric intensive care units

    Objective To analyze research hotspots and development trends in the field of palliative care for pediatric intensive care units abroad,aiming to inform the development of culturally tailored pediatric palliative care models in China. Methods A systematic search was conducted in PubMed and Web of Science for literature on palliative care in pediatric critical care units published from database inception to April 26,2025. CiteSpace version 6.4.R1 was employed to visualize keyword co-occurrence networks,thematic clusters,and emerging terms. Additionally,relevant research publications in China were identified by searching China National Knowledge Infrastructure(CNKI),VIP Database(VIP),and Wanfang Data(Wanfang). Results A total of 502 eligible articles was identified,with a marked upward trend in annual publications peaking at 55 articles in 2024. And 42 Chinese documents were obtained,with the highest annual publication volume reaching 6. International research prioritized on ethical conflicts and moral dilemmas between healthcare providers and families regarding withdrawal of life-sustaining treatments,disease-specific symptom management and end-of-life care for critically ill children,occupational stress and interventions for palliative care teams. The current research trend is shifting towards the continuous family care of terminally ill children. Conclusion Global research on palliative care in pediatric critical care units is intensifying,encompassing ethical decision-making,symptom control,practitioner psychological support,and family grief counseling. These findings offer critical theoretical and practical insights for establishing culturally appropriate pediatric palliative care pathways in China.

    A systematic review of a risk prediction models for subdelirium syndrome in ICU-hospitalized patients

    Objective To systematically search and evaluate the risk prediction models of subdelirium syndrome in hospitalized ICU patients,in order to provide reference for clinical practice and related research. Methods Relevant studies in PubMed,Web of Science,Cochrane Library,Embase,CINAHL,Wanfang Database,CNKI,VIP and CBM were searched systematically until October 12,2024. Two researchers conducted literature screening,information extraction and methodological quality assessment of the literature respectively. Results A total of 8 studies were included,involving 10 models. The sample size ranged from 306 to 700 cases,and the incidence of outcome events ranged from 8.97 to 35.14%. The area under receiver operating curve reported by the model was all >0.7. Calibration degree was reported for 6 models. Three studies performed both internal and external validation. The overall applicability of the eight studies was good,but there was a high risk of bias. Conclusion The study of risk prediction model for ICU patients with subdelirium syndrome is still in its infancy. Future studies should improve the study design and reporting process,and develop a unified subdelirium syndrome evaluation tool to further evaluate the effect and feasibility of the model in clinical practice.

    Research progress and enlightenment on health-related quality of life in out-of-hospital cardiac arrest survivors

    Out-of-hospital cardiac arrest(OHCA) survivors’ health-related quality of life(HRQoL) refers to survivors’ perceptions and evaluations of their health status resulting from the cardiac arrest event and subsequent resuscitation,encompassing physical,psychological,and social adaptation domains. Currently,research on the HRQoL of survivors in China remains relatively limited,and the localized rehabilitation intervention system has yet to be perfected. Therefore,this article reviews international research advancements in the measurement tools and applicability,changes,and determinants of HRQoL among OHCA survivors. Additionally,it proposes three strategic directions for nursing research that integrate both current practical guidance and future development directions,namely developing disease-specific assessment tools,conducting predictive modeling research,and establishing intelligent digital rehabilitation frameworks. This is aimed at expanding related research in China and enhancing HRQoL outcomes for cardiac arrest survivors.

    Application of digital twin technology in the field of acute and critical care:a scoping review

    Objective To conduct a comprehensive review of the application research of digital twin technology in the field of critical care nursing,providing a reference for promoting the digital construction of critical care nursing. Methods Following the methodology of scoping review,a systematic search was conducted in databases such as PubMed,Web of Science,Cochrane Library,CINAHL,Embase,China Biomedical Literature Database,CNKI,Wanfang Database,and VIP Chinese Journal Database for relevant literature on the application of digital twin technology in acute and critical care nursing,the included literature was summarized and analyzed. The search period was from the inception of the database to May 2025. Results 18 articles were included. The application of digital twins in acute and critical care nursing involved disease monitoring and early warning,medical decision support,intelligent medical resource management,medical education,and telemedicine. Evaluation indicators included feasibility evaluation indicators,clinical outcome indicators,and work quality indicators. Conclusion Digital twins can enhance the accuracy,timeliness,and coordination of acute and critical care nursing,improving the quality and efficiency of nursing. Future research should expand the application scenarios of digital twins in clinical settings,establish scientific evaluation indicators,integrate cutting-edge technologies from multiple fields,and promote the development of digital twins in acute and critical care nursing.
