    Design and application of an intra-hospital transport checklist for critically ill cardiovascular patients

    Objective To construct and apply intra-hospital transport checklist for critically ill cardiovascular patients to improve transport safety. Methods The preliminary draft of intra-hospital transport checklist for critically ill cardiovascular patients was developed based on literature review and semi-structured interviews,and the Delphi method was used to conduct 2 rounds of consultation with 12 experts,the final intra-hospital transport checklist for critically ill cardiovascular patients was formed. After evaluating the reliability and validity of the checklist,it was carried out in a cardiovascular surgical ward and a cardiovascular ward of a tertiary class A hospital in Zhejiang Province from September to December 2021. 67 cases of transport from September to October were regarded as control group,72 cases from November to December were experimental group. The experimental group used intra-hospital transport checklist for critically ill cardiovascular patients on the basis of routine transport,while the control group used routine transport nursing. The rate of adverse events and transport time were compared between the two groups. Results The response rate of the 2 rounds of expert consultation were 100%. Expert authority coefficients was 0.811 and 0.856. And expert coordination coefficients were 0.363 and 0.387(P<0.05). Kappa value of each item ranged from 0.733 to 1.000, with average of 0.803(P<0.05). The content validity index at the level of items ranged from 0.810 to 1.000, and the index of whole checklist was 0.912. The checklist contained 6 first-level indicators,22 second-level indicators,and 50 third-level indicators. After application of the checklist,the incidence of some disease/technology-related adverse events, the preparation time and transit time of experimental group were lower than those of control group(P<0.05). Conclusion The construction of transport checklist was scientific and reliable. It can shorten the transport time,and is helpful for the effective implementation of the key link of transport,then improve transport safety.

    Development and psychometric testing of Knowledge-Attitude-Practice Scale on Initiating Do-Not-Resuscitate among emergency medical staff

    Objective To develop Knowledge-Attitude-Practice Scale of Initiating Do-Not-Resuscitate(DNR) among emergency medical staff,and to evaluate the reliability and validity. Methods We formed a pretest scale by literature review and expert consultation. To analyze items of the scale and test the reliability and validity,we processed a survey among 181 medical staff with initiating DNR Knowledge-Attitude-Practice Scale in emergency department of two tertiary hospitals in Zhejiang at August 2020. Participants were selected by convenience sampling. Results The Knowledge-Attitude-Practice Scale on Initiating DNR among emergency medical staff has 31 items and 3 dimensions. There were 10 items in the knowledge dimension,11 items in the attitude dimension,and 10 items in the practice dimension. The variance contribution rate of knowledge,attitude and practice on initiating DNR were 24.86%,21.24%,and 19.33% in order,and the cumulative variance contribution rate was 65.42%. The content validity index(CVI) of the 3 dimensions were 0.962,0.988,and 0.900. The CVI of each item ranged from 0.857 to 1.000. The Cronbach's α coefficients of the scale was 0.953,and the Cronbach's α coefficients of each dimension ranged from 0.919 to 0.940. The total split-half reliability coefficient of the scale was 0.770,and the split-half reliability coefficient of each dimension ranged from 0.843 to 0.909. Conclusion The Knowledge-Attitude-Practice Scale of Initiating Do-Not-Resuscitate(DNR) among emergency medical staff has good reliability and validity,and it can be used to evaluate the initiation of DNR knowledge and behavior level of emergency medical staff in China.

    Objective To construct a training program for nurses in anesthesia intensive care unit,and provide reference for clinical work. Methods Through literature review,group discussion,Delphi expert consultation and analytic hierarchy process,the indexes and weight of nurses' post training program in anesthesia intensive care unit were determined. Results The final training program included 4 first-class items,11 second-class items and 55 third-class items,including inclusion requirement,training contents,training methods and evaluation. The effective response rates of the two rounds of expert consultation were 100%,the expert authority coefficients were 0.913 and 0.928 respectively,and the Kendall harmony coefficients were 0.279 and 0.301 respectively(all P< 0.001). Conclusion The post training program of nurses in anesthesia intensive care unit is scientific and reliable,and can provide guidance and reference for standardized and homogeneous on-the-job training of nurses in anesthesia intensive care unit.

    A study on the construction of a training program for ICU nurses to implement early patient mobility

    Objective To construct a training program for ICU nurses to implement early patient mobility to provide a reference for clinical training. Methods Based on the theory of Knowledge,Attitude and Practice,a Delphi method was used to conduct 2 rounds of correspondence with 21 experts to form a training program for ICU nurses to implement early patient mobility. Results The response rate of the two rounds of experts letter questionnaire was 91.35%(21/23) and 100%(21/21) respectively. The expert authority of the two rounds were 0.921 and 0.912 respectively. The Kendall coordination coefficients of the two rounds were 0.217 and 0.319 respectively. The final constructed training program for ICU nurses to implement early patient mobility included five aspects:training objective,training content,training method,training teachers,and training evaluation,among which the training content included 3 first-level indicators,11 second-level indicators,and 67 third-level indicators. Conclusion This study constructed a scientific,systematic and targeted training program for ICU nurses to implement early patient mobility,which can provide reference for training of ICU nurses to implement early patient mobility.

    Construction and application of simulation course of opioid-induced clinical crisis management for pain nurse in ward

    Objective To construct a simulation course of opioid-induced clinical crisis management for pain nurse in ward and evaluate its effects. Methods Following the best practice standards of International Nursing Association for Clinical Simulation and Learning,according to the five basic dimensions of scenario development,facilitator preparation,equipment preparation,environmental preparation and pre-course student preparation,a opioid-induced clinical crisis management simulation course was constructed. From November to December 2019,a group of 61 pain nurses in wards from a tertiary general hospital in Zhejiang Province were selected for training in the simulation teaching center of the Anesthesia-Operation Department of the hospital. The teaching evaluation was carried out by using the questionnaire of Critical Thinking Disposition Inventory and clinical decision-making self-confidence assessment of excessive sedation and respiratory depression caused by opioids. Results After training,the total score of critical thinking was (264.57± 2.77). There were statistical differences before and after the training(P< 0.05). After training,the clinical decision-making self-confidence of the trainees in the excessive sedation and respiratory depression caused by opioids was(3.89± 0.82) and(3.89± 0.80) respectively,which were statistically different compared with those before the training(P< 0.001). Pain nurses in wards were highly satisfied with the simulation teaching course. Conclusion The simulation course can effectively improve the clinical crisis management ability and clinical decision-making confidence of pain nurses in wards,and can provide reference for the training of comprehensive emergency response ability of pain nurses.

    The effects of the“Trinity” case deductive model training on promoting the emergency response ability of new nurses

    Objective To explore the effects of "Trinity" case deductive group model training on improving the emergency ability of new nurses,in order to provide reference for relieving the impact of transition of new nurses and fastening adaptation to clinical practice. Methods Seventy-two nurses who met the criteria in the second year of standardized induction training were selected as the research objects,and were successively included into the experimental group and the control group according to the order of entry form,with 36 nurses in each group. Both groups received one set of emergency project-themed training each quarter of a one-year cycle. The experimental group was trained in the trinity of "training needs of participants-clinical needs-training teacher's expertise" case deductive group training,the control group received conventional workshop training. After the training,the scores of theoretical knowledge and skills,and evaluations of training teachers and trainees were compared between the two groups. Results The scores of case analysis and nursing assessment ability in the theoretical and skill assessment of the experimental group were respectively (16.18± 2.07) and (19.24± 1.44),compared with the control group (14.34± 2.04) and (17.66± 1.84),the differences were statistically significant (P< 0.05). The excellent rate of skill assessment in the experimental group was 80.56%,which was 13.89% higher than the control group. The experimental group all thought that the extrapolated scene was real and could stimulate their deep thinking. The improvement of self-emergency ability and feedback of training teachers were better than those of the control group,and the differences were statistically significant(P< 0.01). Conclusion The “Trinity” case deductive clinical emer-gency response ability training can effectively improve the new nurses' mastery of the training content,and improve their emergency response ability and psychological state in response to sudden changes of clinical conditions.

    Longitudinal studies of training status of emergency specialist nurses

    Objective To investigate the training status of emergency specialist nurses and provide reference for improving the training of emergency specialist nurses in China. Methods We selected trainees who participate in emergency specialist nurses training by Chinese Nursing Association from 2014 to 2021 and retrospectively analyzed their general information,training status and satisfaction evaluation after training. Results 1 829 trainees from 716 hospitals in 31 provinces,autonomous regions and municipalities in China were investigated. The proportion of male trainees was from 8.78% to 23.33%,with the largest proportion in 2018. The proportion of trainees with intermediate professional titles in 2020 and 2021 was higher than 40%. The annual average proportion of nurses aged from 26 to 35 was 80.92%. The number of trainees with undergraduate degree has reached more than 85% and the master degree has stabilized at more than 2% in the past 2 years. Trainees in East and North China accounted for more than 50% of the total trainees every year. The differences were statistically significant(P< 0.05). The satisfaction of online teaching mode in 2020 and 2021 was lower than that of the offline teaching mode(P< 0.05). And there was no statistical difference in the satisfaction of clinical practice(P> 0.05). Conclusion The training of emergency specialist nurses meets the development needs of the specialty,but the educational background and the number of specialized nurses in northwest,northeast,southwest should be improved. It is recommended to continuously optimize the online and offline hybrid theoretical teaching mode,and promote the formation of a national theoretical and practical training system.

    Correlation analysis between death attitude and hospice education needs of the nurses in neonatal intensive care unit

    Objective To investigate the status quo of nurses' attitude towards death and demand for hospice education in neonatal intensive care unit(NICU),and to analyze the correlation between the two variables. Methods Convenience sampling method was used to select NICU nurses from 5 tertiary hospitals in Shanghai,Hangzhou and Wenzhou as the research objects,and the survey was carried out using the general information questionnaire,Death Attitude Profile Scale and Palliative Care Education Needs Scale. Results A total of 198 NICU nurses were investigated,and the total score of their hospice education needs was (132.19± 20.88) points. The average score of each dimension of death attitude from high to low is neutral acceptance(4.09± 0.56) points,death avoidance(3.28± 0.85) points,death fear(3.07± 0.84) points,approach acceptance(2.71± 0.83) points,escape acceptance(2.71± 0.99) points. The neutral accepting attitude of NICU nurses was positively correlated with educational needs of hospice care(r=0.169~0.524,P< 0.01). Neutral acceptance and negative attitude were included in the influencing factor model of NICU nurses' hospice education needs(F=10.947,P< 0.001),which could explain 21.4% of the variation in hospice education needs. Conclusion NICU nurses have a polarized attitude towards death. The more the nurses tend to have a neutral accepting attitude,the higher the demand for hospice education. Managers should guide nurses towards a neutral attitude towards death,strengthen hospice education,and improve the nursing quality of neonatal palliative care.

    Emergency nursing of a patient undergoing emergency thoracotomy for intracardiac mental foreign body removal

    To summarize the emergency nursing experience of a patient who underwent emergency thoracotomy for intracardiac mental foreign body removal due to accidental swallowing of the suture needle which penetrated into the adventitia of the pulmonary artery. Comprehensive assessment of the risk of major hemorrhage,pneumothorax,and cardiac tamponade should be carried out because the suture needle has broken into the pericardial cavity and was adjacent to the great blood vessels. According to the expert consensus on emergency treatment of esophageal foreign bodies in adults(2020 version),the multidisciplinary diagnosis and treatment plan was quickly launched,the prevention and early identification of bleeding and pneumothorax and standardized transport and handover plans were adjusted in a targeted manner. Close team work allowed the patient valuable time for early surgery. Refinement nutrition management,complication prevention and nursing were the difficult points of postoperative care. After 14 days of treatment and nursing,the patient recovered and was discharged from the hospital. After 3 months of follow-up,the patient was in good condition.

    To summarize emergency care experience of 10 patients with acute ST-segment elevation myocardial infarction complicated by sudden cardiac arrest who underwent extracorporeal cardiopulmonary resuscitation combined with the emergency percutaneous coronary intervention. In view of the characteristics such as the rapid progression of the disease,the difficulty of establishing extracorporeal membrane oxygenation in the case of sudden cardiac arrest,the high risk of percutaneous coronary intervention,the poor postoperative recovery of nervous system function,and the instability of circulation,the caring measures were adopted,including the establishment of a multidisciplinary collaborative emergency care team,precise implementation of cardiopulmonary resuscitation,cooperation with doctors for rapid implementation of extracorporeal membrane oxygenation and percutaneous coronary intervention,temperature management and neurological function monitoring,stabilization of patient hemodynamics after extracorporeal cardiopulmonary resuscitation. Among the 10 patients,five patients' conditions were improved and were discharged,three patients were discharged after giving up treatment due to neurological dysfunction and/or combined multiple organ failure,one patient was transferred to a higher-level hospital for heart transplantation,and one patient died.

    To summarize the emergency nursing experience of a patient with hemorrhagic shock caused by rupture of uterine artery pseudoaneurysm after the cesarean section. Under the conditions including acute onset and rapid progress of the disease,massive bleeding and the short preparation time for the operation,key points of nursing included immediate activation of the emergency nursing team,rescue of hemorrhagic shock and rapid diagnosis,urgent uterine artery embolization,observation of postoperative vaginal bleeding,prevention of infection,prophylactic analgesia to relieve acute ischemic pain of uterus after uterine artery embolization,and relieve of patient's negative emotion by empathy nursing. The patient recovered and was discharged on the seventh day after emergency treatment and careful nursing.

    To summarize the nursing experience of a child with Prader-Willi syndrome complicated with pulmonary infection and secondary septic shock. In view of the rapid progress of septic shock in children,difficulty to effectively remove respiratory secretions,complicated medication,and being prone to complications,nursing points included:the application of specific indicators to effectively identify the signs of shock and close cooperation for rescue between doctors and nurses; bronchoalveolar lavage combined with lung ultrasound to guide chest physical therapy to remove secretions and promote lung recruitment; accurate medication,close monitoring of drug efficacy and side effects; prevention of pressure injury and acute kidney injury. After 30 days of careful treatment and care,the child was discharged in stable condition and recovered well during outpatient follow-up.

    To summarize the nursing care experience for a patient with facial onset sensory and motor neuronopathy. According to the characteristics of type Ⅱ respiratory failure in this patient,multi-disciplinary team cooperation was adopted and noninvasive ventilator was used to correct respiratory failure. Nurses accurately evaluated swallowing function,did a good job in oral hygiene after eating,and implemented measures to prevent aspiration and asphyxia,so as to deal with the risk of aspiration and other risks that may be caused because the patient refused to retain a gastric tube. In view of the rarity of the disease and the patient's wrong cognition and anxiety about the disease,nurses provided personalized list health education,guided the patient to actively deal with the disease. Internet plus online service was also provided for long-term follow-up management. After 7 days of careful nursing,the patient's symptoms were improved and discharged. After 3 months of follow-up after discharge,the patient's symptoms were stable,and there were no adverse events such as aspiration and aspiration pneumonia.

    To summarize the postoperative nursing experience of a case of right pulmonary artery correction supported by extracorporeal membrane oxygenation. In view of the rapid progress of the patient's condition,unstable circulation and being prone to pulmonary arterial hypertension crisis,bleeding and thrombosis,measures included emergent establishment of extracorporeal membrane oxygenation at the bedside,close monitoring of left heart function,adjustment of body position and balance of bilateral pulmonary blood flow,early identification of bleeding signs and active prevention of thrombosis. After 33 days of treatment and nursing,the child was discharged in stable condition. After one year of follow-up,the child was in good condition with normal growth and development.

    To summarize the nursing experience of a patient with type A aortic dissection and grade Ⅳ difficult airway who underwent aortic dissection surgery and uvulopalatopharyngoplasty. In view of the patient's hemodynamic disorders,volume disorders and other problems,we maintained hemodynamic stability and volume dynamic balance. For acute respiratory insufficiency,individualized lung protective ventilation measures were adopted to prevent ventilator-associated pneumonia. According to the nursing difficulties and emphases of the airway with grade Ⅳ difficulty after aortic dissection,emergency artificial airway preplan was established to prevent unplanned extubation and throat bleeding,and sequential tracheal intubation extubation was implemented. The patient was given a risk hierarchy based on his high risk of delirium,early stage activities were enhanced. After 31 days of intensive care,the patient recovered well and was discharged.

    Construction and application of the closed caregivers closed-loop management scheme of the ward under the pandemic prevention and control

    Objective To explore the best caregivers management scheme of general wards under the normalization of the epidemic situation,and to effectively prevent nosocomial cross-infection on the basis of ensuring the caregivers needs of patients. Methods We formulated the targeted plan by analyzing the problems existing in the epidemic control. Face input,electronic caregivers card,double access control system and access scanning and registration system were used to carry out closed closed-loop management of caregivers,supplemented by pre-hospital education,supervision during hospitalization,logistics and material support and discharge services implementation. Compared the caregiver rate,average daily caregivers rate,per capita exit time,access closed-loop rate,daily caregiver management hours before and 3 month,6 month after implementation. Compared the patient satisfaction and the quality control score of ward environment 11 months before and after implementation of the plan. Results The caregivers rate decreased by 15.72%,the double caregivers rate decreased by 77.8%,the average daily caregivers rate decreased by 36.6%;the per capita caregivers exit time decreased by 19.7 min,the closed loop rate increased by 100%,daily caregivers management time decreased by 17.6 min 3 month after the implementation of the scheme. There were significantly differences before and after implementation(P< 0.05). After 6 month of the program,the caregivers rate decreased by 9.90%,the double caregivers rate decreased by 0,the average daily caregivers rate decreased by 44.8%;the per capita caregivers exit time decreased by 13.0 min;the closed-loop rate was 100%;the daily caregivers management time decreased by 26.6 min compared with those before implementation(P< 0.05). No nosocomial cross-infection events occurred. Ward nursing environment scores and patient satisfaction had no statistical differences before and after the implementation(P> 0.05). Conclusion The implementation of the closed caregivers closed-loop management scheme in the general ward under the normal epidemic situation is feasible,which can effectively reduce the caregiver rate,reduce the flow of caregivers,prevent the hospital cross-infection.

    Objective To explore the design and clinical application effects of information management system for high-risk patients with difficult airway. Methods The information management system was designed and used for assessment,information input,warning,recording,automatic identification in patients with previous difficult airway. Results The information management system for patients with difficult airway can automatically identify and provide early warning of patients with previous difficult airway,realize the sharing of patient information among different departments,to instruct doctors and nurses to deal with patients with difficult airway timely and accurately,and to optimize nursing workflow. A total of 21 patients with hign-risk difficult airway were recognized and warned from 705 inputted patients over a 6-month period,and no adverse events occurred. Conclusion The application of information management system for high-risk patients with difficult airway can promote the management efficiency of patients with previous difficult airway,ensure the airway safety of patients and improve the medical quality. The active participation of the nursing team is the basis of promoting the comprehensive and effective application of the system.

    Clustering analysis on research hotspots of Acinetobacter baumannii in ICU

    Objective To analyze the research hotspot and development trend of Acinetobacter baumannii in ICU. Methods The literatures related to Acinetobacter baumannii in ICU were retrieved from CNKI and Web of Science database until January 2022,and the literatures were visualized by CiteSpace 5.8.R3 software. Results A total of 928 English literatures and 2 194 Chinese literatures were included. The number of Chinese literatures showed a rapid growth trend from 2008,and the number of English literatures increased rapidly from 2014. Cluster analysis showed that clusters of ICU,severe pneumonia,drug resistance,bacteria,drug resistance,drug resistance rate,infection,nosocomial infection,risk factors and Antibacterial drugs were formed in Chinese literature,and the research hotspots were tegecycline,drug resistance genes and influencing factors. In English literature,gene,colistin,Gram negative bacteria,antimicrobial resistance,Acinetobacter baumanii,contamination,ventilator-associated pneumonia,antibiotic-resistance and bacteria were nine clusters. The research focus is the treatment and management of Acinetobacter baumannii infection. The main content of highly cited literature is the definition of Acinetobacter baumannii and its related introduction. Conclusion In recent years,the number of literature on Acinetobacter baumannii infection in ICU continues to increase,and the research focus is on the epidemiological investigation,drug resistance mechanism,prevention and control,and nursing measures of Acinetobacter baumannii infection in ICU. Since there is no optimal treatment plan for Acinetobacter baumannii infection at present,more nursing research can be carried out on the management and prevention and control measures of hospital infection in the future.

    Handover was the transfer of the patient's clinical data to another medical staff or professional team,including the discussion of the patient's special information and the transfer of medical and nursing responsibilities. Environmental interference,complicated conditions of children,cultural differences among medical staff,and differences in clinical specialty might affect the comprehensiveness of handover information,and standardized tools should be used to guide communication. This article reviewed four commonly used handover tools for pediatric patients. The focus of each handover tool was different,and it was difficult to meet the handover needs in different situations. It was recommended to choose the tool according to clinical needs.

    Childlike care is a combination of psychological content,infancy and childlikeness throughout the hospitalization process of children,in order to improve children's treatment compliance and improve the quality of life. This article reviews the application progress of childlike nursing in critically ill hospitalized children at home and abroad,including the overview of childlike nursing,the form of intervention and its application effects. However,the evaluation and intervention forms of children at different ages need to be further improved.
