日间肺癌手术患者出院时恐惧疾病进展的潜在剖面分析及影响因素研究

A latent profile analysis and influencing factors of fear of progression at discharge from day-surgery among lung cancer patients

摘要:

目的 探究日间肺癌手术患者出院时恐惧疾病进展的潜在剖面类别及影响因素,为实现个性化、分层次心理干预提供参考。方法 便利抽样法选取2023年10月—2024年2月浙江省某三级甲等医院225例日间肺癌手术患者。采用一般资料调查表、肺癌患者术后症状量表和恐惧疾病进展量表进行调查。通过潜在剖面模型识别恐惧疾病进展类别,多因素Logistic回归分析探究不同剖面与基线、症状的关联。结果 调查对象共分为“高恐惧—个人感受组”(n=17,7.56%)、“中恐惧—疾病困扰组”(n=98,43.55%)、“低恐惧—疾病适应组”(n=110,48.89%)3个类别,影响因素包括头晕、疼痛、恶心呕吐、失眠、皮下气肿(P<0.05)。结论 日间肺癌手术患者在出院时对疾病进展的担忧存在差异。护理人员应关注个体特征,动态识别并管理影响因素,加强出院指导与健康管理,改善患者就医体验。

Abstract:

Objective To explore fear of progression profiles and related factors at discharge in lung cancer day surgery patients,and guide individualized hierarchical psychological interventions. Methods Convenience sampling was used to select 225 patients undergoing day surgery for lung cancer,who were admitted to a tertiary-level A hospital in Zhejiang Province from October 2023 to February 2024. The survey adopted the general information questionnaire,the Postoperative Symptom Scale for lung cancer patients,and the Fear of Progression Questionnaire-Short Form. Latent profile analysis was used to identify categories,and multivariate Logistic regression was used to examine the associations between different profile groups,baseline characteristics and symptoms. Results There were three categories:“high fear-personal feelings group”(7.56%,n=17),“moderate fear-disease distress group”(43.55%,n=98),“low fear-disease adaptation group”(48.89%,n=110). Influencing factors included dizziness,pain,nausea and vomiting,insomnia,and subcutaneous emphysema(P<0.05). Conclusion Day surgery lung cancer patients show varying levels of concern about disease progression at discharge. Nursing staff should focus on individual characteristics,dynamically identify and manage related factors,strengthen discharge guidance and health management to improve the medical experience.
