Nursing for living-donor liver transplantation in a child with recurrent hepatoblastoma complicated by invasive pulmonary aspergillosis

摘要：

总结1例复发肝母细胞瘤合并侵袭性肺曲霉菌感染、先天性支气管肺发育不良及不完全性右束支传导阻滞患儿行亲体肝移植的围手术期护理体会。护理要点包括：基于肺保护理念的呼吸道管理，术后即刻拔除气管插管并序贯经鼻高流量氧疗；目标导向的血流动力学监测与液体管理；医护药协同的免疫抑制与抗真菌治疗监护；环境—操作—患儿人文支持三位一体的感染防控。通过上述综合护理干预，患儿术后32 d顺利出院，随访5个月一般情况良好。

Abstract:

This article summarized the perioperative nursing experience for a pediatric patient undergoing living donor liver transplantation，who presented with recurrent hepatoblastoma complicated by invasive pulmonary aspergillosis，congenital bronchopulmonary dysplasia，and incomplete right bundle branch block. Key nursing interventions included airway management based on lung-protective strategies，immediate postoperative extubation followed by sequential high-flow nasal oxygen therapy；goal-directed hemodynamic monitoring and fluid management；collaborative monitoring of immunosuppressive and antifungal therapy by nurses，physicians，and pharmacists；a tripartite infection prevention and control approach focusing on environment，procedures，and personnel. Through these comprehensive nursing interventions，the child was discharged successfully 32 days after surgery and remained in good general condition at the 5-month follow-up.
