摘要:

目的 探讨移动医患交互平台在心脏瓣膜置换术围手术期患者吸气肌训练(inspiratory muscle training,IMT)中的应用效果。方法 采用便利抽样法,选取2023年8月—2024年6月于浙江省某三级甲等医院心脏大血管外科行心脏瓣膜置换术的患者124例为研究对象,采用随机数字表法分为试验组和对照组,试验组患者采用联合医患交互平台的IMT,对照组采用常规呼吸功能锻炼。比较两组患者在出院时及出院1个月门诊复查时的衰弱水平、心肺功能、肺部并发症等指标。结果 最终纳入研究对象122例,试验组和对照组各脱落1例。试验组患者在出院1个月后的衰弱水平、心肺功能和肺部并发症发生率均优于对照组,差异有统计学意义(P<0.05),并且衰弱评分、握力、5次坐立试验、肺功能指标等存在交互效应(P<0.05)。结论 基于移动医患交互平台的IMT能改善心脏瓣膜置换术后的心肺功能,逆转衰弱状态,降低术后并发症发生率,为术后居家康复的延续护理提供实践路径。

Abstract:

Objective To explore the efficacy of a mobile patient-physician interactive platform for perioperative inspiratory muscle training(IMT) in patients undergoing heart valve replacement. Methods Using the convenient sampling method,a total of 124 patients who underwent cardiac valve replacement in the cardiovascular surgery department of a tertiary grade A hospital in Zhejiang Province from August 2023 to June 2024 were selected by convenience sampling and randomly divided into an experimental group and a control group using a random number table. The experimental group received IMT combined with a mobile patient-physician interaction platform,while the control group received conventional respiratory function exercise. Cardiopulmonary function indicators,frailty levels,cardiopulmonary function and postoperative pulmonary complications were compared between the two groups at discharge and during the one-month follow-up outpatient visit. Results A total of 122 patients were collected in this study,and one dropout each in the experimental and control groups. Patients who received mobile platform-based IMT showed significantly better frailty levels,cardiopulmonary function,and postoperative pulmonary complications at the one-month follow-up compared to the control group(P<0.05). Additionally,interaction effects were observed in frailty score,grip strength,Five-Times Sit-to-Stand Test(FTSTS) and pulmonary function indicators(P<0.05). Conclusion IMT delivered via a mobile patient-physician interaction platform improves cardiopulmonary function,reverses frailty,and reduces postoperative pulmonary complication rates in patients following heart valve replacement,offering a novel practical pathway for continuity of care during home-based rehabilitation.
