Abstract:

This article summarized perioperative nursing for a patient with a rare giant retroperitoneal leiomyosarcoma involving the lumbar spine who underwent latissimus dorsi turnover flap transplantation after massive intraoperative hemorrhage. To balance circulatory instability and flap perfusion, triple monitoring (vital signs, urine output, flap) and goal-directed fluid therapy were implemented within 72 h. Multimodal quantitative flap perfusion assessment,a graded emergency protocol, meticulous positioning management, multidimensional vasospasm prevention, and design of a fenestrated lumbodorsal brace were applied. The patient was discharged after 15 days with a favorable prognosis at the 6-month follow-up.
