劳力型热射病院前降温护理的最佳证据总结

Summary of the best evidence on prehospital cooling care for exertional heat stroke

摘要: 目的 评价和总结劳力型热射病院前降温护理的最佳证据,为临床开展劳力型热射病院前降温护理提供依据。方法 系统检索BMJ Best Practice、UpToDate、苏格兰学院间指南网、英国国家临床医学研究所指南网、美国指南网、澳大利亚乔安娜布里格斯研究所循证卫生保健中心数据库、新西兰指南协作组、加拿大安大略注册护士协会、加拿大临床实践指南网、中国实践指南注册平台、Nursing Consult、Cochrane Library、PubMed、中国生物医学文献数据库、中国知网、万方数据库、医脉通中劳力型热射病院前降温护理的证据,包括指南、专家共识、临床决策、系统评价等。检索时限为建库至2022年12月31日。由2名研究者进行文献质量评价和证据提取。结果 共纳入10篇文献,包括指南1篇、专家共识8篇、系统评价1篇,从早期识别与诊断、脱离热环境及确保呼吸循环、快速准确监测核心体温、积极有效快速降温4个方面总结了21条证据。结论 劳力型热射病院前降温护理证据总结可为临床医护人员或急救人员提供参考,有助于降低劳力型热射病所致的器官损伤率及病死率。

Abstract: Objective To evaluate and summarize the best evidence of pre-hospital cooling care for exertional heat stroke(EHS)and provide basis for clinical pre-hospital cooling care. Methods We systematically searched BMJ Best Practice,UpToDate,Scottish Colleges Guideline Website,National Institute of Clinical Medicine,National Guideline Clearinghouse of USA,Joanna Briggs Institute Library,New Zealand Guidelines Collaborative,Registered Nurses' Association of Ontario,Canada Clinical Practice Guidelines,China Practical Guidelines Registration Platform,Nursing Consult,Cochrane Library,PubMed,China Biomedical Literature Database,CNKI,Wanfang and Yi Maitong for evidence on prehospital cooling care for exertional heat stroke,including guidelines,expert consensus,clinical decision-making and systematic review. The retrieval time limit was from the establishment of the database to December 31,2022. The literature quality evaluation and data extraction were performed by two researchers. Results A total of 10 articles were included,including one guideline,eight expert consensuses and one systematic review. Finally,21 pieces of best evidence were summarized from 4 aspects. Conclusion The summarized nursing evidence of EHS prehospital cooling can provide reference for clinical medical staff or emergency personnel,and help to reduce the organ damage rate and mortality caused by EHS.
