小肠移植受者再次行肝肠联合移植的术后护理

Postoperative nursing care of a recipient of small bowel transplantation underwent second liver-intestine transplantation

摘要/Abstract

摘要： 总结1例小肠移植受者再次行肝肠联合移植的术后护理体会。针对患者术后排斥反应缺乏临床特异性、急慢性肾功能衰竭及感染发生率高等问题,术后严密监测排斥反应,血浆置换期间做好各项监测,预防抗体介导的排斥反应;实施精准化的液体管理方案,维持肾脏功能;全程落实保护性隔离措施,积极预防感染;制订个体化的康复计划,并给予分阶段式个体化营养支持,促进整体机能恢复。经过114 d的积极治疗和护理,患者转至普通病房康复治疗,3个月后康复出院。随访11个月,患者状况良好。

Abstract: To summarize the nursing points of intestinal transplantation recipient who underwent combined liver-intestinal transplantation. According to the lack of rejection clinical specificity,acute and chronic renal failure and high incidence of infection in patients after operation,rejection was closely monitored,all kinds of monitoring were performed during plasma exchange to prevent antibody mediated rejection; implement precise fluid management to maintain kidney function; protective isolation measures were developed to actively prevent infection during the whole process; individualized rehabilitation and nutrition support were given to promote recovery. After 114 days of active treatment and nursing,the patient was transferred to the general ward for rehabilitation,and recovered 3 months later. 11-months follow-up after discharge showed that the patient was in good condition.
