    Clinical practice and analysis based on Intrahospital Transport Consensus for Emergency Critical Patients

    Since the release of the Consensus on Intrahospital Transport for Emergency Critical Patients in 2017, it has been widely applied and implemented in China. It provided rules to follow and realized standardized and national consistent transport. Through clinical practice and analysis in the past two years,standardized transport process can guide the clinical work consistently. Based on the general framework of transport,different specific transport modes are developed and applied widely. However,the transport of patients with special groups,such as ECMO patients,critically ill pregnant women,children and infants,needs to be further studied and discussed to meet clinical needs. Therefore,intrahospital transport of emergency critically ill patients needs an effective and systematic standard construction and management for clinical high-risk steps. It is a long-term and frequent clinical practice which needs to be highly emphasized and continuously improved by clinical staff.

    Emergency triage is the first gateway for emergency patients. The quality of triage is often related to the treatment outcome of patients. There is no unified and detailed emergency triage standard in our country since now. Triage is still based on the subjective experience of emergency department nurses in the most of hospitals.National Health Commission of the People’s Republic of China organized this project,and the Second Affiliated Hospital of Zhejiang University School of Medicine took the lead in drafting the standard of emergency triage,which established a detailed and reliable emergency triage scale for emergency nurses in 2018. This is the adult part of the emergency triage scale.

The emergency triage principle,triage level and standard,triage nurse requirements were interpreted to guide the emergency triage for nu ses in China and provide basis for triage training and management.

    The unified and standardized skin care norms and procedures for incontinence patients provide clinical standard guidance for nursing staff,and are also the guiding basis for reducing the incidence of skin complications of patients. In 2022,the Third Medical Center of the General Hospital of the People’s Liberation Army collaborated with 16 hospitals nationwide to write the group standard of the Chinese Research Hospital Association “skin care norms for adult incontinence patient (T/CRHA 016-2023)”,which was released in January 2023. It mainly standardized the overall evaluation of skin care for adult incontinence patients,the selection of nursing products,the standard process and the key points of nursing. This article explained the important parts and items in this standard in detail to facilitate users’ understanding and promote the standardized practice of skin care for patients with clinical incontinence.

    The impact of two methods of extubation onthe occurrence of extubation complications

    Objective To compare the impact of positive pressure extubation and negative pressure extubation on extubation complications in patents with successful spontaneous breathing test. Methods 48 patients with successful spontaneous breathing trial in a first-class level hospital from July 1 ,2019 to December 31 , 2019 were enrolled and were divided into positive pressure ex tu bation group(25 cases) and negative pressure extubation group(23 cases) using a random number table. End Expiratory Lung Impedance changes (delta EELI % )was a primary observation index. Secondary observ ation indexes included breathing rate ,heart rate , mean arterial pressure, blood oxygensaturation changes and upper airw ay complications which included airway hy perresponsiveness (wheezing in the larynx, new wheezing in both lungs) ,severe coughing , and respiratory distress. Results In the positive pressure extubation group , EELI% decreased by (14.84+2.03)% after extubation, and all EELI returmned to the level before extubation within an average of one minute. In the negative pressure extubation group, the EELI% decreased by(3 1.09+4.64)% after extubation,the average time for EELI to return to the level before extubation was 3 minutes ,and the EELI within 30 minutes after extubation in 4 patients was still lower than the level before extubation. The difference between the two groups was statistically significant ( P<0.05). The number of complications occurred within 30 minutes after extubation was compared between the two groups. Shortness of breath occurred in 5 cases vs.15 cases in positive pressure vs. negative pressure group. HR increased by 20%(4 cases vs.11 cases), MAP increased by 20%(2 cases vs.8 cases) ,SpO2 decreased by more than 5%(2 cases vs.8 cases) ,and SpO2 decreased by more than 5%(2 cases vs.8 cases).Upper airway complications included airway hyperresponsiveness(1 vs.6 cases) , severe cough (3 vs.9 cases),and gastric reflux(1 vs.6 cases )and respiratory distress(2 vs.10 cases).The differences of complications were statistically significant between two groups (P<0.05 ). Conclusion Positive pressure extubation can effectively reduce the pulmonary collapse caused by extubation and reduce the incidence of upper airway complications.

    The pregnant and maternal women are the special group and the  adult emergency triage  scale can't be directly applied for them. It is highly necessary to establish an obstetric-specific emergency triage scale which is suitable for the  pregnant and  maternal  women.  The  obstetric  emergency  triage  scale  is  a part  of the  emergency triage  scale organized  by the  National  Health  Commission of the  People's  Republic  of  China and  drafted  by the leading of the Second Affiliated Hospital of Zhejiang University School of Medicine. The scale includes the most of common obstetric emergency conditions and details the classification indicators of each level of obstetric conditions.It provides an objective,quantitative and reliable standards for clinical triage nurses.
