摘要

总结1例高处坠落致截瘫术后并发急性肺栓塞患者在体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)支持下取栓治疗的护理体会。护理要点:密切关注心源性休克后心功能的恢复情况、加强体外膜肺氧合辅助支持期间的护理、实施序贯性抗凝管理、制订气道大出血应急预案及实施阶段性整体康复训练方案。经过综合监护室13 d的治疗与护理,患者病情稳定,顺利转回骨科病房进一步康复治疗。

Abstract

To summarize the nursing experience of thrombus removal under the support of extracorporeal membrane oxygenation(ECMO) in a patient with postoperative acute pulmonary embolism after paraplegia caused by fall from height. The main nursing points included paying close attention to the recovery of cardiac function after cardiogenic shock,strengthening the nursing during ECMO support,implementation of sequential anticoagulation management,formulation of emergency plans for massive airway bleeding,and implementation of phased holistic rehabilitation training programs. After 13 days of treatment and nursing in the comprehensive intensive care unit,the patient‘s condition was stable and she was transferred back to the orthopaedic ward for further rehabilitation treatment.
