摘要

总结1例先天性孤立肾合并重症肺炎患儿在体外膜肺氧合与俯卧位通气支持下的护理体会。护理要点：多学科协作实现院外体外膜肺氧合高效启动与安全转运；限制性液体复苏优化肾脏保护；电阻抗断层成像技术引导俯卧位通气促进肺康复；感染防控联络护士主导切断感染源头全程管控；家庭参与的多感官护理改善心理状态。患儿经30 d的积极治疗及精心护理，多器官功能恢复，顺利转至当地医院进一步康复治疗。

Abstract

To summarize the nursing experience of a child with congenital solitary functioning kidney and severe pneumonia supported by extracorporeal membrane oxygenation and prone ventilation. Key nursing interventions included multidisciplinary collaboration to achieve efficient initiation and safe out-of-hospital transport of ECMO，restrictive fluid resuscitation to optimize renal protection，prone ventilation guided by electrical impedance tomography（EIT） to support staged pulmonary rehabilitation，infection control led by liaison nurses for infection prevention and control to cut off infection source and whole process management，and family-involved multisensory care to promote psychological recovery. After 30 days of active treatment and comprehensive nursing，the patient’s organ function gradually recovered and she was successfully transferred to a local hospital for further rehabilitation.
